Mental Health in Lockdown: The World From My Window

This year’s Mental Health Awareness Week (18 – 24th May) comes at an unprecedented time of global change.

As nations all across the planet slowly unfurl the restrictions of lockdown and societies begin to take small steps back to a version of ‘normal’ life, the lack of certainty over what will happen next continues to cause anxiety and distress to many.

During the lockdown, The World From My Window (TWFMW) has shone a spotlight on the stories of the individuals who are living through this difficult, shared experience. So far, hundreds of personal accounts and stories have been collected from 40 different countries across the globe; including, Pakistan, Uganda, North America, Sudan, the UK, Belarus, New Zealand and Palestine.

Everyone has a different story to tell. We have heard from patients in the throes of recovery, parents at home – struggling to balance demands of children and work and popstars – including Little Mix’s Leigh-Anne, who is finding hope in the opportunity to stay still.

What has been the common thread in all the submissions and stories received, is that making the time to write down your anxieties and emotions can bring comfort during a period of crisis.

You can add your voice to the growing digital anthology of stories told by all these global citizens by writing a short story or letter to describe what your world looks like right now. The goal is to gather stories from as many countries and societal perspectives as possible and to build a varied and holistic picture of this moment in our shared history.

During Mental Health Awareness Week, TWFMW is encouraging more submissions through a partnership with Centrepoint and a series of interactive Instagram live workshops and discussions.

Isolation is especially hard on vulnerable young people and TWFMW is feeding into the Centrepoint, #ChallengeAccepted, arts initiative by giving young people an opportunity to share their story via social channels in a series of different tasks and focuses to keep active and engaged during the lockdown.

Health and social services staff to get free access to mental health apps

  • Daylight and Sleepio apps available to everyone working in health and social services in Scotland to support their mental health during the crisis
  • Staff can download mental health apps for free to help with sleep and anxiety during COVID-19 pandemic
  • Announcement part of nationwide drive to digitise healthcare, starting with workforce mental health

All health and social services staff in Scotland can now access free apps to cope with insomnia and anxiety as part of a nationwide effort to support the mental health of key workers during the COVID-19 pandemic. 

NHS Scotland has this week launched a national programme to provide those working in health and social services with access to practical, everyday advice and support, as well as mental health apps and tips on self-care to help staff cope with the challenges they are facing with the pandemic.

As part of the PRoMIS National Wellbeing Hub, the apps on offer include programmes designed to help maintain healthy and restorative sleep, reduce anxiety and worry, and to help staff learn evidence-based techniques to address mental health issues when they arise.

The NHS-accredited apps Daylight – designed to help with worry and anxiety – and Sleepio – a digital sleep improvement programme featuring Cognitive Behavioural Therapy (CBT) techniques – are both developed by digital therapeutics company Big Health.

Sleepio has already been made available to almost 30,000 residents in the Western Isles, and both Sleepio and Daylight were recently made  available at no cost to all NHS employees across England thanks to a partnership with NHS England.

As the UK’s response to the COVID-19 pandemic continues, health and social services staff remain at the frontline and are being placed under mounting pressure. The news comes as first Minister Nicola Sturgeon has stressed that the “stay at home” message remains in place in Scotland after Boris Johnson announced his “conditional plan” to reopen society.

Big Health co-founder and Professor of Sleep Medicine at the University of Oxford, Professor Colin Espie, has praised NHS Scotland’s timely response in protecting the mental health of NHS staff as well as their physical health during the pandemic.

Commenting on Big Health’s partnership with NHS Scotland Professor Espie said: “I’m honoured that Big Health is able to support our colleagues in NHS Scotland to help safeguard their mental health during these most challenging of times.

“NHS staff are working under enormous pressure whilst also experiencing significant changes to their lives at home as a result of COVID-19. We built Sleepio and Daylight to help people effectively manage their mental health whether the challenges occur during the night or day.”

Dr Sandra Ferguson, Associate Director Psychology and Lead for National Trauma Training Programme at NHS Education for Scotland added: ““There is increasing evidence that insomnia and sleep difficulties is an area that staff in health and social care are increasingly reporting.

“We are delighted to have worked with Sleepio who have designed this evidence-based app to support staff in resolving these difficulties. A good night’s sleep is a central part of overall wellbeing.”

Sleepio and Daylight are available at no cost to everyone working in the NHS to support their mental health during the COVID-19 pandemic.

These solutions aim to support staff to maintain healthy and restorative sleep, reduce anxiety and worry, and to help them learn evidence-based techniques to address mental health issues when they arise.

WHO: Substantial investment needed to avert mental health crisis

The COVID-19 pandemic is highlighting the need to urgently increase investment in services for mental health or risk a massive increase in mental health conditions in the coming months, according to a policy brief on COVID-19 and mental health issued by the United Nations today.

“The impact of the pandemic on people’s mental health is already extremely concerning,” said Dr Tedros Adhanom Ghebreyesus, Director-General of the World Health Organization. “Social isolation, fear of contagion, and loss of family members is compounded by the distress caused by loss of income and often employment.”

Depression and anxiety are increasing

Reports already indicate an increase in symptoms of depression and anxiety in a number of countries. A study in Ethiopia, in April 2020, reported a 3-fold increase in the prevalence of symptoms of depression compared to estimates from Ethiopia before the epidemic.

Specific population groups are at particular risk of COVID-related psychological distress. Frontline health-care workers, faced with heavy workloads, life-or-death decisions, and risk of infection, are particularly affected. During the pandemic, in China, health-care workers have reported high rates of depression (50%), anxiety (45%), and insomnia (34%) and in Canada, 47% of health-care workers have reported a need for psychological support.

Children and adolescents are also at risk. Parents in Italy and Spain have reported that their children have had difficulties concentrating, as well as irritability, restlessness and nervousness. Stay-at-home measures have come with a heightened risk of children witnessing or suffering violence and abuse. Children with disabilities, children in crowded settings and those who live and work on the streets are particularly vulnerable.

Other groups that are at particular risk are women, particularly those who are juggling home-schooling, working from home and household tasks, older persons and people with pre-existing mental health conditions. A study carried out with young people with a history of mental health needs living in the UK reports that 32% of them agreed that the pandemic had made their mental health much worse.

An increase in alcohol consumption is another area of concern for mental health experts. Statistics from Canada report that 20% of 15-49 year-olds have increased their alcohol consumption during the pandemic.

Mental health services interrupted

The increase in people in need of mental health or psychosocial support has been compounded by the interruption to physical and mental health services in many countries. In addition to the conversion of mental health facilities into care facilities for people with COVID-19, care systems have been affected by mental health staff being infected with the virus and the closing of face-to-face services. Community services, such as self-help groups for alcohol and drug dependence, have, in many countries, been unable to meet for several months.

“It is now crystal clear that mental health needs must be treated as a core element of our response to and recovery from the COVID-19 pandemic,” said Dr Tedros Adhanom Ghebreyesus. “This is a collective responsibility of governments and civil society, with the support of the whole United Nations System. A failure to take people’s emotional well-being seriously will lead to long-term social and economic costs to society.”

Finding ways to provide services

In concrete terms, it is critical that people living with mental health conditions have continued access to treatment. Changes in approaches to provision of mental health care and  psychosocial support are showing signs of success in some countries.

In Madrid, when more than 60% of mental health beds were converted to care for people with COVID-19, where possible, people with severe conditions were moved to private clinics to ensure continuity of care. Local policy-makers identified emergency psychiatry as an essential service to enable mental health-care workers to continue outpatient services over the phone. Home visits were organized for the most serious cases.

Teams from Egypt, Kenya, Nepal, Malaysia and New Zealand, among others, have reported creating increased capacity of emergency telephone lines for mental health to reach people in need.

Support for community actions that strengthen social cohesion and reduce loneliness, particularly for the most vulnerable, such as older people, must continue. Such support is required from government, local authorities, the private sector and members of the general public, with initiatives such as provision of food parcels, regular phone check-ins with people living alone, and organization of online activities for intellective and cognitive stimulation.

An opportunity to build back better

“The scaling-up and reorganization of mental health services that is now needed on a global scale is an opportunity to build a mental health system that is fit for the future,” said Dévora Kestel, Director of the Department of Mental Health and Substance Use at WHO.

“This means developing and funding national plans that shift care away from institutions to community services, ensuring coverage for mental health conditions in health insurance packages and building the human resource capacity to deliver quality mental health and social care in the community.”

Edith Bowman voices recording to boost mental wellbeing while walking

A mood-boosting podcast to help Scots unwind while they walk has been released to the public.

The ‘Mind to Walk’ podcast, narrated by DJ and presenter Edith Bowman, talks you through a walk that will relax your mind as you move your feet.

Launched by Scotland’s walking charity, Paths for All, the audio is designed using mindfulness techniques to help people look after their mental wellbeing whilst getting their daily exercise during Covid-19.

During periods of social distancing, isolation and uncertainty, many have expressed increased feelings of anxiety, stress or worry. Daily outdoor exercise, whilst adhering to social distancing guidelines, has become an important way to look after physical and mental wellbeing.

The podcast uses Edith’s familiar voice to help the listener bring their mind and body together whilst enjoying 25 minutes of self-care.

Launched as part of May’s National Walking Month, the campaign aims to support people dealing with stress and anxiety caused by Covid-19, during their once daily outdoor exercise as recommended by the government.

Ian Findlay CBE, Chief Officer with Paths for All, hopes that people of all ages will give the podcast a listen, and believes the calming benefits of the ‘Mind to Walk’ campaign can be far-reaching.

He said: “The power of a short daily walk has become even stronger recently. It’s a time for us to wander outside, get some fresh air and leave our worries behind. With the podcast, we’re hoping to make that burst of activity even more beneficial by helping people tune into their surroundings.

“The podcast is expertly scripted and Edith’s comforting and recognisable voice will help any listener thoroughly de-stress and clear their head, to remove some of the tension a lot of us are feeling.

“I’d encourage you to try this frequently on your daily walks as evidence shows that the effects of mindfulness are greater the more you practice. It might feel a little awkward to start with, but the script is designed to be listened and enjoyed by everyone, regardless of experience in using mindfulness techniques.”

Written by Glasgow-based meditation expert Dassini Cadwallender, ‘Mind to Walk’ has been designed to be used on any walking route – whether it is around a housing estate or in a nearby woodland, listeners will finish feeling calmer and ready to enjoy their day.

The voice behind the audio, Edith Bowman is delighted to be involved and said: “This is such a strange time for us all and I think being aware of your physical and mental health is so important as we move forward into the unknown.

“When I heard about ‘Mind to Walk’ I was eager to be involved. If I can do one small thing to help a few folk’s lockdown less anxiety-ridden it makes me very happy. If we can get as many people as possible taking the podcast on their walks with them, that would be incredible, and hopefully helpful to them.”

Paths for All believes regular walking is key to leading a happy and healthy life – and it is important for people to continue enjoying short, local walks where it is safe and appropriate to do so, owing to the huge array of associated benefits.

With around one in three people estimated to be affected by mental health problems in Scotland in any one year,this is set to be compounded during the pandemic – with a further 20% reporting that they are worried about mental illnesses directly related to Covid-19, including 11% who are concerned about anxiety and 7% who have concerns about depression.

The impact of the increased pressure can also manifest in different ways, with 38% saying that they have slept less or worse than they normally do. Physical activity has been proven to reduce the risk of depression by 30%.

The script for the ‘Mind to Walk’ recordings acts as guided walking meditation, allowing listeners to connect with their bodies, nature and the sensation of walking as well as stretching and relaxing as they walk.

The script has been designed to be listened and enjoyed by everyone, regardless of their experience in practicing mindfulness. It has been found the effects of mindfulness are greater the more individual’s practice.

More mental health support after forty days of lockdown

Key mental health services to support families, young people and autistic people are to receive more than £1 million additional funding.

Health Secretary Jeane Freeman made the announcement yesterday after 40 days of lockdown when many people have felt an effect on their mental health.

The allocation of funding comprises:

• £768,000 for a relationships helpline to be delivered by The Spark counselling service
• £105,000 to support Young Scot to develop enhanced digital content and resources on mental health
• £205,000 to support 47,000 autistic people across Scotland, including funding to increase capacity at the Scottish Autism Helpline and help for the National Autistic Society to keep people in touch online during lockdown

Ms Freeman said: “While everyone is experiencing different challenges at this time, the mental health of children, young people and families can be put under great strain.

“We appreciate that spending a lot of time together in isolation and home-working, home-schooling and juggling childcare can be really tough.

“This crisis means many people are dealing with anxiety, fear, uncertainty, anger or sadness. They may feel overwhelmed and relationships can be placed under a lot of pressure.

“We want to ensure that the right help and support is in place for the mental wellbeing of our children, young people and families. This package of measures focuses on prevention and recovery, and on maintaining healthy relationships throughout the current restrictions. It follows the launch of our national campaign, Clear Your Head, last week.”

The Clear Your Head campaign highlights practical ways to look after mental health and wellbeing while continuing to stay at home – and signposts sources of help and advice.

The Spark is a third sector organisation which delivers counselling services to families across Scotland, including relationship and couples counselling and counselling for children and young people.

They currently operate a Relationship Helpline on a small scale for nine hours a week. The number of the Relationship Helpline is 0808 802 2088 and this additional funding will enable the helpline to operate Monday-Thursday 9am to 9pm and Friday 9am to 4pm.

Scotland has launched a digital resource called Mind Yer Time specifically to help children and young people learn about the healthy use of screens and social media.

It supports mental and physical health online and was developed by the Scottish Youth Parliament and Children’s Parliament. In the first ten days since its launch the guide had almost 13,000 views.

The additional funding will allow the Scottish Autism Helpline to open 8am-8pm, seven days a week on 01259 222022.

Greens raise concerns around home-care support

Lothian MSP Alison Johnstone has called on the region’s Health and Social Care Partnerships to urgently address concerns around the reduction in provision of home-care support, which has left many disabled people in an extremely difficult position.

Reports suggest that the Lothian region’s Health and Social Care Partnerships have reduced their visits by 356 in total, leaving many vulnerable people struggling to cope – a matter the Scottish Greens raised in parliament this week.

Alison Johnstone MSP said: “The figures published this week raise major concerns, There is huge disparity around the country, with the City of Edinburgh Health and Social Care Partnership reducing their clients by 240, East Lothian by 44, Midlothian by 24, and West Lothian by 48, while at the same time Angus increased its by 80.

“The Scottish Government confirmed that extra support is available and that if more is needed it will be provided, so our Health and Social Care Partnerships must confirm that they will immediately access this support and restore home care visits.

“I’m extremely concerned about the impact of these changes, which result in some people having their support packages dramatically reduced, and others losing support entirely, with many then forced to rely on family members for personal care, meals and medication.

“The impact on the mental wellbeing of disabled people, particularly in terms of social isolation which can disproportionately affect disabled people, is a huge concern. While its vital that care is restored as quickly as possible, the immediate roll-out of online or remote mental health resources would be really valuable right now.”

New fund for perinatal mental health

Mums, dads and families at risk of poor mental health during pregnancy and the early years will be able to access enhanced peer support, parenting support and perinatal counselling services, with new annual funding of up to £1 million.

The Perinatal and Infant Mental Health fund will provide crucial help by supporting third sector organisations to deliver key services and address gaps in provision.

Inspiring Scotland will run the fund – which will open to charities from August – and will work with the Scottish Government to achieve the goals of the Perinatal and Infant Mental Health Programme Board.

Minister for Mental Health Clare Haughey said: “The third sector provides an invaluable source of support for the mental health of women, babies and their families during pregnancy and early parenthood.

“Last year, the Scottish Government provided funding to support the work of third sector organisations in this area and I am delighted that we are able to build on this with the new Perinatal and Infant Mental Health Fund.

“The fund will allow one-to-one support for organisations to help provide the services that women and their families need across Scotland.”

Inspiring Scotland Chief Executive Celia Tennant said: “We are delighted to be supporting the Scottish Government with this important new third sector fund for Perinatal and Infant Mental Health charities.

“We know how vital the perinatal period is for families and how crucial the early years are for infant development. Charities across Scotland are doing essential work, and with the uncertainty caused by coronavirus (COVID-19), their work is more important now than ever.

“We look forward to continuing our support of the third sector and their support for infants and families at this crucial stage of life.”

The application process for the Perinatal and Infant Mental Health Fund will open on 5 May 2020.

The fund will run for three years, from August 2020 – March 2023.

Additional mental health support announced

Additional support to help people look after their mental health and wellbeing during and after the coronavirus (COVID-19) pandemic has been announced by First Minister Nicola Sturgeon.

The support includes an investment of more than £1 million towards the expansion of the Distress Brief Intervention (DBI) programme to help people in distress, and the launch of a new mental health marketing campaign across television, radio, print and online.

The DBI programme, which was previously operating in four pilot areas, will roll-out across Scotland, giving people over the age of 16 who are in emotional distress due to COVID-19 the opportunity to speak to specially trained staff. People who are in distress but do not need clinical intervention will be referred to the DBI programme by frontline staff, including NHS 24.

The first phase of the new mental health campaign will launch this month and provide people with practical advice on coping with the current restrictions. Signposting to existing advice will be included and those who need extra support will be directed to NHS Inform as a key information resource and helplines operated by NHS 24, Breathing Space, SAMH and Samaritans.

The First Minister said: “COVID-19 restrictions have affected all of our lives. People who may never have been affected by mental health issues in the past, are now facing emotional distress due to financial loss, bereavement and social distancing. 

“NHS 24 has experienced an increase in calls, including to Breathing Space, in recent weeks and we anticipate this will continue to grow. It is vital that during this period of uncertainty anyone who requires support for their mental health can receive it.

“Since 2017 more than 6,600 people have accessed the DBI programme and I am pleased that people across the country will now be able to access this support. Early intervention like this is such an important part of how we treat mental and emotional health and the DBI is all about equipping people with the skills and support to manage their own health and to prevent future crisis.

“This £1 million investment is in addition to the £3.8 million we invested last month to increase capacity of our telephone and digital mental health services. This national marketing campaign will equip us all with some practical things we can do to feel better and help us cope until things return to normal.”

The DBI programme, which launched in 2017, currently operates in Lanarkshire, the Borders, Inverness and Aberdeen.

Around £1,038,000 will now be invested in recruiting staff to expand and develop the Distress Brief Intervention programme nationwide.

Funding will be distributed to NHS Lanarkshire and NHS Borders, third sector partners Penumbra, Support in Mind and SAMH, and to Stirling University for evaluation and Glasgow University for training.

The nationwide DBI programme will mean that people in distress related to COVID-19 who have no need for clinical intervention will be either linked directly by phone to NHS 24 by the first responder, or directed to phone NHS 24 if they are able to. Once through to NHS 24 an initial assessment will be made as to next steps, and whether they should then be referred to the DBI programme. If they are referred the distress responder will follow up with a phone call within 24 hours of referral.

Voiceover artists say: Keep Talking

Over 70 male, female and child voiceover artists have donated their voices to a specially-made radio commercial to encourage listeners to ‘keep talking’ whilst the UK is in lockdown.

“Voiceovers United” is a project created, written and produced by Voiceover Artist and award-winning Radio Commercial Producer, John Calvert. The message encourages listeners to ‘stay at home, protect the NHS, stay safe… And Keep Talking.”

John Calvert said: “Voiceovers often spend hours and hours cooped up in a small booth or room.  Most of us in the trade are perfectly used to that. But for those who aren’t so used to isolation, the long lonely lockdown could have adverse effects on mental health. We all know talking is a great way to get things off your chest and who better to promote this than voiceovers?”

Some of the UK’s leading voiceover artists have joined the project. These include X Factor voice Peter Dickson; ‘Voice of the Balls’ Alan Dedicoat; film trailer voice Redd Pepper; Big Brother Voiceover Marcus Bentley; Flash Gordon, Blackadder and Star Wars character actor Brian Blessed and many more familiar voices from the world of TV, radio, film, advertising, gaming, animation and automation. Everyone who participated, including voiceover agents, animation companies and music publishers gave their services for free.

The voiceovers were sent a script and recorded the lines in their own studios. The audio was then sent back to the studio at Airforce Radio Commercial Production in Wiltshire and seamlessly mixed together.

John Calvert added: “So many leading voices uniting for something like this has never been done before, and the response to the initiative was far beyond anything I expected.

“What we have ended up with is a very powerful, yet entertaining message. I hope everyone will feel reassurance from hearing the message, share it with others and … Keep Talking.”

Voiceover Artist Emma Clarke (voice of ‘mind the gap’ and ‘Avalon’ in the Hollywood blockbuster feature film ‘Passengers’) said: “During this unprecedented period, we’re all alone together.

“I love the idea of lone voices coming together to create a whole in this gorgeous piece of audio. It reflects what’s happening in the world – that we all have to play our part as individuals to help the whole world.

“Everyone I speak to is feeling the same – tired, scared, overwhelmed, angry, powerless… and also joyful, connected, needed, loved, hopeful. We’re all feeling the same vulnerabilities. There’s something very powerful in that shared experience and for me, that’s what this piece of audio is about.”

The Voiceovers United project is inviting radio stations across the UK to broadcast the commercial at any time they see fit. It is available to download for free on Soundcloud.

One Year No Beer steps up to support frontline workers

Local Edinburgh start up, One Year No Beer, has sacrificed salaries to ensure a free service to support NHS staff, frontline support workers, all key workers, the unemployed and anyone who needs additional help in taking a break from alcohol during this challenging time.

The bold decision follows reports that confirm alcohol sales in the UK are up 22% in March.

 Ruari Fairbairns, founder and CEO of One Year No Beer, said: “We are currently experiencing the worst global pandemic of our lifetime, which has caused two-thirds, if not more, of adults across the UK to experience anxiety.

“The major problem here is that people are using alcohol as a coping mechanism and this is probably the worst thing that they could do because it is a depressant. We want to help people to look after their mental health and live a better life.”

The brand’s plea follows a recent statement from a World Health Organisation (WHO) expert who warned that alcohol is an “unhelpful coping strategy” for the possible stress and isolation of coronavirus lockdown.

 Fairbairns continued: “The NHS is under increased pressure and this is only set to worsen in the next few weeks as we hit the peak, so in addition to this, the last thing we want people to do is incur any other alcohol-related health issues resulting in an urgent trip to hospital.

“Reducing your alcohol intake and making positive decisions about your health are both great ways to help keep the NHS safe and protect others, all from the safety of your home.”

 Recent research has also revealed that there has been a huge rise in mental health calls, as well asGP’s highlighting an increase in domestic violence and according to step counters, steps are down 50-100% inEurope, suggesting that as a result of COVID-19, people are exercising less and drinking more.

Professor Kevin Mooreone of London’s leading liver specialists with more than 35 years of experience comments: “There is so much positive research and work going on right now in the fight against COVID-19, but it’s vital that people don’t overlook the hugely negative impact that alcohol could have during these challenging times.
