Campaigners call on Scottish Government to develop national strategy to prevent child sexual abuse

Stop It Now! Scotland and NSPCC Scotland are calling on the Scottish Government to develop a national strategy to tackle child sexual abuse that focuses on prevention to make the country a safer place to grow up.

The two charities hosted an event in partnership with the Scottish Parliament this week (Wednesday, March 15) with leading experts in the field.

They discussed the devastating harms and long-lasting impact that sexual abuse can have on victims, that punishment alone will not eradicate this problem and what we can do to prevent children being abused in the first place. They also talked about the next steps we need to take to guarantee Scotland is the safest country for children to grow up.

They are urging the Scottish Government to develop a comprehensive and coordinated national approachto prevent child sexual abuse, which involves health, police, education, community safety, children’s services, social services, housing and the wider community. The child protection charities say it is vital that everyone understands what child sexual abuse is and knows how they can be part of preventing it.

A review of UK data revealed that 15 per cent of females and 5 per cent of males will experience some form of sexual abuse before the age of 16.

This means at least 80,000 children in Scotland will have been affected by this issue before they leave high school. Although this is thought to be an under-representation of the scale of the problem and the actual number of children who have experienced sexual abuse in Scotland is not known.

The charities say that to understand the numbers of children in Scotland affected and the scale of the suffering it is crucial that a prevalence survey is conducted.

Professor Elizabeth Letourneau, Director of the Moore Centre for the Prevention of Child Sexual Abuse, School of Public Health, John Hopkins University, said: “Child sexual abuse affects about one in nine children – 12 per cent of all children – globally. Victims are at risk of immediate harms, such as fear, injury, and pregnancy, and a broad array of serious health problems can emerge and last across the lifespan.

“Only one in five cases of child sexual abuse are ever reported to the authorities. This means that we miss at least 80 per cent of cases, so punishment will never be enough if we are to effectively address this public health problem.

“I believe our failure to focus on prevention stems from a general misgiving that child sexual abuse really is not preventable. That people who are at risk of perpetrating abuse are monsters and their behaviour cannot be predicted or prevented, and they will only respond to punishment.

But we know this is not true. We already have good evidence that we can effectively prevent child sexual abuse perpetration.”

Pat Branigan, Assistant Director of NSPCC’s Together for Childhood, said: “One of the most important messages from today is that child sexual abuse is preventable and not inevitable.

“The ultimate goal is to develop a framework, based on evidence of what we know already works, which can be used to support agencies and organisations to work together and prevent child sexual abuse in communities.

“We need to create strong local partnerships between social care, schools, health, voluntary groups, the police and communities that focus on preventing people from offending, and empower and educate children and adults to recognise the signs of abuse and how they can report their concerns.

“Ultimately it will not be governments, experts or professionals who eradicate child sexual abuse, it will be individuals, families and communities.”

Stuart Allardyce, Director, Lucy Faithfull Foundation / Stop It Now! Scotland, said: “Last year we helped 7,000 people across the UK through our Helpline.  Not all were adults worried about their own sexual thoughts, feelings and behaviour, but around half of them were.

“We also had adults who were worried about another adult, or because they had found evidence their partner has been seeking out child sexual abuse material online.

“We believe that if you build prevention initiatives, people will come – professionals, protective adults, but also those who worried about the risk that they may present to children. We can no longer say that people won’t use self-help prevention resources, because the evidence is that they can and do and that prevention works.”

Childlight, based at the University of Edinburgh, also launched the first comprehensive global data repository this week, which will look at all forms of child sexual exploitation and abuse (CSEA), with the aim of being able to show the scale and nature of this abuse.

Anyone with concerns about a child’s wellbeing can contact the NSPCC Helpline on help@nspcc.org.uk. The NSPCC practitioners provide free and confidential help and advice and can take appropriate steps to help keep children safe. If a child is in immediate danger, please call 999.

The NSPCC also has advice and resources for parents, carers on how to have simple, age appropriate conversations with children to help prevent sexual abuse through their Talk PANTS campaign. This helps children understand that their body belongs to them and to recognise when something is not okay and how to tell someone.

NHS Lothian releases results of research into transmission of COVID-19 from hospitals into care homes

Scientists in NHS Lothian have helped to develop national infection controls after studying COVID-19 transmission between hospitals and care homes.

The teams found that 99 per cent of patients discharged from hospital into care homes during the first wave of the pandemic did not introduce COVID-19 into care homes.

The study, conducted with the University of Edinburgh, focused on patients in Lothian discharged from hospital into 130 care homes from 1st March 2020 to 31st May 2020. 

Of the 787 discharges in this time, the researchers found that the majority – a total of 776 – did not introduce COVID-19 into care homes as a result of their hospital stay.

Forty-one of the discharged patients had a positive COVID-19 test within two weeks of leaving hospital and genome sequencing was used to identify if these cases could be linked.

The study found that 30 patients likely contracted this in the care home or from the community.

The results of 10 patients were inconclusive and one patient was confirmed to have contracted COVID-19 during their stay in hospital.

Very sadly, this patient later passed away and a number of residents in their care home subsequently contracted COVID-19.

Dona Milne, Director of Public Health and Health Policy, NHS Lothian, said: “This study has shown that 99 per cent of hospital discharges didn’t introduce COVID-19 cases into Lothian care homes during the first wave of the pandemic.

“However, we’re able to say with certainty that one care home outbreak in this time originated from us. We are deeply saddened by this, and our heartfelt apologies and condolences are with the families and loved ones of those affected.

“We began testing symptomatic care home residents very early in the pandemic, so it’s been really important for us to use this information to find out what we can learn about how the virus was contracted and spread and how this linked to our hospitals.”

Before the nation went into its first lockdown when testing wasn’t widely available, NHS Lothian built capacity into its labs specifically to PCR test those who were symptomatic in hospitals and care homes.

Because of this early testing of symptomatic patients, in-depth data was available to the research team to enable them to genome sequence the transmission of the virus as part of the study.

Dr Kate Templeton, Head of Molecular Diagnostics for NHS Lothian who led the study, said: “Testing was a priority for us from the beginning and we were the first health board in Scotland to build capacity in our labs specifically for care home residents.

“This early testing has enabled us to genome sequence the virus and track where this was acquired and how this spread, leading to the creation of an outbreak methodology which has informed best practice at both Scotland and UK-level.

“This methodology not only has applications to COVID-19 but other viruses too and has been vital in learning lessons around infection control. It also clearly shows that the protection of the most vulnerable in society needs to involve not just health authorities but the community too.”

The theme for World Sleep Day yesterday (17th March) was ‘Sleep is Essential for Health’. Poor sleep can negatively impact our heart health, along with many other areas of our physical and mental wellbeing.

Prioritising your seven to nine hours shut eye every day is one of the best things you can do to support your health. During sleep, our body gets to work helping us to recover both mentally and physically, which helps us feel energised, alert, and ready for the day.

Sleep isn’t something to overlook, so here are some tips for helping to improve your sleep:

Limit caffeine

Who doesn’t love a morning cuppa? Whilst a small amount of caffeine early in the day is unlikely to impact our sleep, caffeine can stay in our blood stream for many hours after consumption.

This can negatively impact the quality of our sleep and ability to fall asleep. So, say no to caffeine after lunch!

Turn off the screens

Melatonin is our sleepy hormone, and we need to allow our body to produce lots of it to ensure a good night’s sleep. Bright lights in our environment can reduce the production of melatonin, specifically the blue light from computer screens, tablets, and mobile phones.

Try switching off screens an hour before sleep time and read a book or have a bath instead.

Spend time outdoors

Research shows that simply being inside during the day, rather than spending time outdoors, reduces our melatonin production at night by 50%!

Grab your trainers and hit the pavement for a walk or run outdoors for at least 20 minutes every day.

Create a routine

Humans are creatures of habit, and we thrive on routine. Creating a bedtime ritual can help signal to our bodies that it’s time for sleep.

Try to choose low stimulation activities like reading, listening to music, or even having a cup of (caffeine free!) tea such as camomile.

As the UK gig economy shows no signs of slowing, The Royal Society for the Prevention of Accidents (RoSPA) has issued a warning to Scottish businesses and workers about driving when tired.

In the gig economy, instead of a regular wage, workers get paid for the ‘gigs’ they do, such as food delivery or a car journey. In the UK, it’s estimated that over five million people are employed in this type of capacity.

Drivers and couriers who get their work from apps face a ‘heightened risk’ of collisions, with almost half (42 per cent) of drivers reporting vehicle damage because of a collision, according to research from University College London.

Tiredness from overwork was flagged as a key risk for those delivering food and parcels, while 63 per cent reported to have not been given the appropriate safety training on managing risks on the road.

One in 10 reported someone had been injured in a crash while they had been working, with scores more reporting time pressure as a key factor in their driving.[1] 

Karen McDonnell, Head of RoSPA Scotland, said: “With the cost of living crisis continuing to bite, it’s easy to see how those in the gig economy could be overworked and tired. We know that being tired is a huge risk factor when driving, and so it’s easy to see how the roads could get more dangerous.

“While it can be easy to say someone should cut down their hours, it’s not always as simple as that. Responsibility lies largely with the companies employing gig workers, who often have unrealistic targets and are expected to go above and beyond to deliver.”

What to do if you feel tired when driving or riding

  • When you first start to feel tired, stop and take a break
  • Stop in a safe place – make sure you’re not on a motorway hard shoulder
  • If you can, drink some coffee or another caffeinated drink
  • Take a short nap, ensuring your car is in a secure location.

What to do if you’re driving for a long distance

Even experienced HGV drivers are stopped from driving more than nine hours a day. Many drivers are not used to driving anywhere near this long, so always bear in mind:

  • Don’t drive for more than eight hours in a day
  • Take regular breaks, aiming to stop every two hours or so
  • On very long journeys, try and plan overnight breaks if you can
  • Don’t begin a long journey if you are feeling tired.

International Long COVID Awareness Day: 15th March

A recent ONS survey showed that an estimated 2.3 million people in the UK suffer from long COVID.  Of these, nearly 10 thousand NHS workers are currently absent because of long COVID, according to a recent BBC panorama documentary (writes FOYSOL CHOUDHURY MSP). 

Sadly, many of these key workers have contracted Covid-19 through their essential work to save lives during the worst of the pandemic, having been put more at risk by working on the frontline, often without adequate PPE.  

Now, a far cry from the weekly applauses and hero’s welcome given during the COVID-19 lockdowns, key workers with long COVID have sadly been seemingly abandoned and forced to face severe financial impact due to contracting COVID-19 in their line of work. 

It is simply unacceptable that these same key workers who risked their lives are now in a position where they are being forced to declare themselves bankrupt or to sell their house, as they face the prospect of halved and altogether stopped standard sick pay. 

This is the situation my constituent Cass Macdonald is in. On Tuesday, I spoke in my colleague, Jackie Baillie’s, debate on long COVID to highlight the impact of long COVID on my constituents like Cass. 

Cass was a full-time NHS nurse who contracted COVID-19 when they were advised it was safe to work in an office nursing role, despite multiple underlying health conditions and advice by the same Government to shield. 

Shockingly, the Keyworker Petition UK team’s recent survey showed that Cass is part of the 20% of key workers with long COVID surveyed who said they were at risk of losing their home due to financial circumstances. The survey also showed that 68.7% of those surveyed said they were struggling financially. This is simply unacceptable. But not only are key workers with long COVID facing unacceptable financial burden-they also have to battle with a health service which does not properly understand the complex nature and consequences of long COVID. 

Another of my constituents, Marie-Claire, who is an NHS doctor with long COVID, told me that there was growing stigma towards long COVID in the medical community and that her colleagues often had little understanding about the severity or specifics of the illness. When key workers are having to battle long COVID and the severe financial impact of contracting this condition, they should not also have to face the frustrating situation of having to educate the public, politicians and medical professionals on this debilitating condition. 

On Wednesday, Cass and other key workers will deliver a petition with over 118, 000 backers to Westminster, urging the UK Government to create a pension and compensation scheme for all keyworkers who have developed long COVID as a result of their frontline, and often not adequately protected, work during the pandemic.  

As noted in my speech on Tuesday, the Scottish Government has a role to play too. Not only must it do more to ensure adequate support and compensation for key workers who are struggling with long COVID, but it has an important role to play in reducing the stigma around long COVID and ensuring that health authorities in Scotland have the tools to adequately identify and treat long COVID in patients. Recent figures suggest that up to 175, 000 people in Scotland have reported suffering from long COVID, which is approximately 3.3% of the population.

The Scottish Government must ensure employers are supported in recognising the importance of flexibility and phased returns to work for employees with long COVID. It must do more to increase public awareness of the dangers of long COVID and highlight how you could be affected by this despite being vaccinated.  

TODAY 15th of March is International Long COVID Awareness Day. I’ve lodged questions to the Scottish Government on support for key workers with long COVID before and now I am imploring the Scottish Government again, in recognition of this day, to act now to help people like Cass, Marie-Claire and the thousands of others suffering from long COVID across Scotland.

Understanding child sexual abuse, and what causes it, is a vital step towards developing effective prevention strategies.”

Scotland, said: “We are calling on the Scottish Government to carry out a dedicated prevalence survey to identify the levels of sexual offending against children in Scotland, alongside a national strategy for tackling child sexual abuse.

“We know that sexual abuse has an enormous impact on the mental health outcomes for children and adult survivors and the economic costs for the criminal justice system.

“We believe sexual harm towards children and young people could be reduced if we focused more on prevention, including therapy being offered to adults who are worried about their sexual thoughts and feelings towards children.”

The international team, made up of experienced individuals from academia, the private sector, law enforcement and non-governmental organisations, will produce an annual report, global dashboard and index with the data that can be used by tech companies, law enforcement agencies and those who work in child protection so they can take action to prevent CSEA.

Safety withdrawal of pholcodine-containing cough and cold medicines

Following a comprehensive scientific safety review of the latest available data on the safety of pholcodine, together with advice from the independent Commission on Human Medicines, it has been concluded that the potential risks outweigh the benefits for these products.

The Medicines and Healthcare products Regulatory Agency (MHRA) keeps the safety of medicines under review and acts where necessary to protect patients and the public.

Following a comprehensive scientific safety review of the latest available data on the safety of pholcodine, together with advice from the independent Commission on Human Medicines, it has been concluded that the potential risks outweigh the benefits for these products.

There is already a known link between pholcodine and an increased risk of the very rare event of a severe allergic reaction (anaphylaxis) to muscle relaxants (neuromuscular blocking agents) that are used during general anaesthesia in surgery. In September 2022 the findings of a new study further strengthened the evidence of this link, but the overall risk is very small.

The MHRA advises that pholcodine-containing cough and cold medicines are being removed from the UK market as a precautionary measure.

Dr Alison Cave, MHRA Chief Safety Officer said: “Safety is our top priority, and we keep the safety of medicines under continual review.

“Following a thorough scientific safety review of all the available evidence on pholcodine, together with advice from the independent Commission on Human Medicines, it has been recommended, as a precautionary measure, that these products should no longer be used.

“If you are taking a cough or cold medicine, check the packaging, label or patient information leaflet to see if pholcodine is a listed ingredient. If it is, and you have any questions, you can talk to your pharmacist who will suggest an alternative medicine.

“If you have an operation scheduled using general anaesthesia, tell your anaesthetist if you think you have used pholcodine, particularly in the past 12 months. The anaesthetist will be well placed to take this into account. Anaesthetists are highly trained in managing anaphylaxis.

“If you want more information or advice, please speak to your pharmacist.”

Figures from leading vet charity PDSA, which provides vital care for pets during the cost of living crisis, have revealed that 32% of owners in Scotland are having to make personal sacrifices to ensure they can continue to provide for their pets.

PDSA provides free and low-cost treatment to pets in need and has released the data to raise awareness of the lengths pet owners are having to resort to while navigating the cost of living crisis.

19% of Scottish pet owners are reducing how much fuel they use so they can pay for their pets care1. This comes as Google searches for ‘save money on heating’ spiked by a shocking 878% in 2022.

Having juggled expensive bills throughout winter, alongside the soaring cost of living, owners may face a further hit this April. While the energy price cap is being reduced, meaning the amount suppliers can charge goes down, government help – in the form of the energy price guarantee – is set to come to an end. This means a household’s energy bills could increase by around £3,000 per year.

The looming spring Budget announcement also isn’t expected to go a long way in easing the strain on UK pet owners’ pockets. The huge hike in everyday living costs means pet owners will continue to struggle with the cost of loving their companions.

Giving up personal luxuries (19%) and cancelling or not going on holidays (9%) are among the other sacrifices owners in Scotland are making to save as much as possible in order to continue caring for their pets.1

39% of Scottish owners are worried about affording the cost of treatment if their pet should fall ill or be injured – making PDSA’s support crucial. Nationally, a quarter of all pet owners (26%) said they’d go into debt3, either with family and friends or via credit and loans, to cover the cost of unexpected veterinary treatment1.

Yet, with 95% of people are determined to do whatever possible in order to avoid having to make the agonizing decision of rehoming them or having them put to sleep – primarily due to the ever-increasing costs to live1.

PDSA Veterinary Surgeon, Lynne James, said: “Everyone wants the best for their pets and hearing the lengths loving owners in Scotland are having to go to so they can continue providing for them is heart-breaking.

“In 2022 we provided veterinary care for over 390,000 pets, whose owners would otherwise have struggled to afford the cost. Now more than ever, the treatment we provide is a lifeline for families who face the horrible decision of eating regular meals or treating their furry family member.

“It’s PDSA’s mission to keep people and pets together. Last year we helped hundreds of thousands of families. With more than half of those who rely on our services aged 55 and over, and 37% disabled or living with a serious health condition, their pet often provides vital companionship. For lots of our clients, their pets are their only companion, and their lives would be unimaginable without them.

“I’d encourage anyone struggling to afford the cost of veterinary treatment to find out if they are entitled to access our services by visiting the eligibility checker on our website. We also have lots of free advice on how to reduce the cost of caring for pets, while ensuring they remain healthy and happy.”

The charity has warned these unsettling findings reflect the stark reality for pet owners, many of whom are being forced to make drastic cutbacks as they desperately struggle to stay afloat.

A worrying 21% of owners in Scotland are cutting back on their weekly food shop, and 2% even admit to going without necessities, such as skipping meals to afford the costs associated with looking after their beloved companions1.

PDSA provides free and low-cost treatment to pets in need and has released the data to raise awareness of the lengths pet owners are having to resort to while navigating the cost of living crisis.

19%1 of Scottish pet owners are reducing how much fuel they use so they can pay for their pets care1. This comes as Google searches for ‘save money on heating’ spiked by a shocking 878% in 2022[ii].

Having juggled expensive bills throughout winter, alongside the soaring cost of living, owners may face a further hit this April. While the energy price cap is being reduced, meaning the amount suppliers can charge goes down, government help – in the form of the energy price guarantee – is set to come to an end. This means a household’s energy bills could increase by around £3,000 per year.

The looming spring Budget announcement also isn’t expected to go a long way in easing the strain on UK pet owners’ pockets. The huge hike in everyday living costs means pet owners will continue to struggle with the cost of loving their companions.

Giving up personal luxuries (19%) and cancelling or not going on holidays (9%) are among the other sacrifices owners in Scotland are making to save as much as possible in order to continue caring for their pets.1

39%1 of Scottish owners are worried about affording the cost of treatment if their pet should fall ill or be injured – making PDSA’s support crucial1. Nationally, a quarter of all pet owners (26%) said they’d go into debt3, either with family and friends or via credit and loans, to cover the cost of unexpected veterinary treatment1.

Yet, with 95% of people are determined to do whatever possible in order to avoid having to make the agonizing decision of rehoming them or having them put to sleep – primarily due to the ever-increasing costs to live1.

iSIMPATHY: Improved patient care and safety

A cross-border trial has improved care for patients prescribed multiple medicines.

The iSIMPATHY project, funded by the European Union’s INTERREG VA Programme, worked with professionals in Scotland, the Republic of Ireland and Northern Ireland to comprehensively review patient medication.

Taking multiple medicines can be problematic if the increased risk of harm from interactions between drugs, or between drugs and diseases, outweighs the intended benefits.

Interim findings showed these interventions potentially prevented major organ failure, adverse drug reactions, avoided hospital admissions and saw patients moved to more appropriate medication.

Project funding, managed by the Special EU Programmes Body (SEUPB), was match funded by the Northern Ireland Executive, the Irish Government and the Scottish Government.

Scotland’s Public Health Minister Maree Todd said: “This project looked at some of our most vulnerable patients taking more than five medications. The reviews have avoided adverse combinations of drugs and hospitalisations while also reducing prescriptions and drugs costs.

“We will know more when the full evaluation is published in June, we will work with partners to see how we can these improvements can be applied more widely, potentially saving lives and money.”  

Ireland’s Minister for Health, Stephen Donnelly said: “Medicines are the most common healthcare intervention used within the health system, and the use of the right medicine for the right patient at the right time is central to this.

“In the delivery of this project, pharmacists were strategically and ideally placed as medicines experts within a multidisciplinary team framework working to maximise therapeutic outcomes for optimal patient benefit.

“I’d like to thank all the partners involved in the iSIMPATHY project for their work to achieve this.”

“iSIMPATHY has achieved this through training pharmacists and other healthcare professionals to deliver medicine reviews and embed a shared approach to managing multiple medicines.  Northern Ireland has been delighted to collaborate with our partner regions to build on the success of previous projects dedicated to improving medicine safety.”
