Latest suicide figures are ‘the tip of the iceberg’, says charity

Data from National Records of Scotland (NRS) released yesterday shows that there were 833 probable suicides registered in Scotland in 2019, an increase from 784 in 2018.

Six hundred and twenty men and 213 women took their own lives in 2019, with people aged 45-54 most at risk. People in our poorest areas were also more likely to take their own lives.

This link between deprivation and suicide is well known. Between 2015 and 2019, people living in the most deprived areas were three times more likely to die by suicide compared to those living in the least deprived areas.

Claire Sweeney, Director of Place and Wellbeing at Public Health Scotland said: “Each of these deaths are a tragedy arising from profound distress, and are all preventable. They will have left immeasurable grief across families and communities.

“The unprecedented challenges we have all faced in 2020 perhaps make it easier to understand that anyone can experience mental illness. That’s one of the reasons why Public Health Scotland is prioritising improving mental wellbeing as part of our strategic plan (A Scotland where everybody thrives).

“We will work with our partners to help grow the understanding of levels of suicide in the communities they work with, and more broadly to support better mental wellbeing across Scotland.”

Responding to Public Health Scotland figures revealing that 833 people died by probable suicide in 2019, a spokesperson for the Scottish Children’s Services Coalition commented: “The statistics highlighting that there were 833 probable suicides registered in Scotland in 2019 is devastating for so many families and only the tip of the iceberg when it comes to highlighting the mental health crisis we are currently in.

“We have for some time called for radical transformation in our mental health services and expressed our concerns over the lack of investment in these for our children and young people. Currently around 50p in every £100 of NHS spending goes to child and adolescent mental health services (CAMHS), and yet we know that one in ten children has a diagnosable mental health problem and that figure is increasing. These problems of course if not addressed carry on into adult life.

“With increased numbers of those experiencing mental health problems, as well as those whose existent conditions are being worsened due to the coronavirus pandemic, coupled with further cuts in services, this points to a mental health crisis ‘perfect storm’.

“While referrals have dropped during lockdown and children are not accessing support, we are storing up immense problems for the future as mental health services face being overwhelmed due to greatly increased demand.

“In addition to the NHS, local authorities, the third sector and other independent organisations play a key role in providing mental health support. Yet these services have seen their resources cut over the years, putting an even greater burden on specialist and costly mental health services. It is vital that they receive increased funding or we face witnessing a ‘lost generation’ of vulnerable children and young people

“Mental health services must be given funding and resources equal to that of physical health provision and this pandemic has ushered in an urgency to deliver on this which cannot be ignored.”

Government welcomes the MHRA review into Pfizer and BioNTech vaccine

UK’s independent regulator to evaluate latest data from Pfizer and BioNTech to ensure the quality, safety and effectiveness of the vaccine meets the standards required.

  • The UK has ordered 40 million vaccine doses from Pfizer/BioNTech – enough for up to a third of the population
  • Initial data shows the vaccine is 94% effective in protecting people over 65 years old from coronavirus and clinical trials have not reported any serious safety concerns

The UK Government has today (Monday 23 November) welcomed the Medicines and Healthcare products Regulatory Agency’s (MHRA) review of data from Pfizer/BioNTech to determine whether its vaccine meets robust standards of quality, safety, and effectiveness.

The companies have reported data that indicates their vaccine is 94% effective in protecting people over 65 years old from COVID-19, with trials suggesting it works equally well in people of all ages, races and ethnicities.

As the first country to pre-order the vaccine from Pfizer/BioNTech, the UK is expected to receive a total of 40 million doses by the end of 2021, enough to vaccinate up to a third of the population, with the majority of doses anticipated in the first half of next year.

The COVID-19 vaccine will only be authorised for supply by the UK’s independent regulator the MHRA if it meets strict standards of quality, safety, and effectiveness, and if they are satisfied the vaccine can be consistently manufactured.

Today marks the next step forward for the vaccine following the MHRA’s confirmation that it has received the necessary data to progress their review into whether the vaccine meets the required standards.

Despite encouraging data about this vaccine, people must continue to follow public health advice to keep themselves and their loved ones safe; regularly washing their hands, wearing a face mask and making space.

Health and Social Care Secretary Matt Hancock said: “The whole country will be cheered by the news that Pfizer/BioNTech have formally reported the data from their clinical trials for their vaccine to the regulator.

“This is another important step on the road to recovery. We must now allow the MHRA’s renowned teams of scientists and clinicians to make an independent assessment of whether it meets their robust standards of quality, safety, and effectiveness.

“If approval is granted, the NHS will be ready to deliver. The NHS has vast experience in delivering widespread vaccination programmes and an enormous amount of work has taken place to ensure we have the logistical expertise, transport and workforce to roll out a vaccine according to clinical priority, at the speed at which it can be manufactured.”

Business Secretary Alok Sharma said: “Today, we have renewed hope that we are on the brink of one of the most significant scientific discoveries of our time, as we reach the crucial last stage to finding a COVID-19 vaccine.

“While this news is a cause for celebration, we must make sure that this vaccine, like all new medicines, meets standards of quality, safety, and effectiveness. I urge the public to be patient while we wait for regulators to do a thorough assessment.

“Finding a vaccine is not going to end the pandemic overnight, but we are hopeful of being one step closer to defeating this terrible virus.”

Final deployment plans will depend on decisions by regulators but preparations have been underway for months and are in place to ensure that:

  • the NHS will be ready to begin vaccinating as soon as the first vaccine is approved and delivered to the UK
  • GPs have already been signed up to take part in the programme when an appropriate vaccine is ready
  • dozens of hospitals across the country will lead co-ordination on behalf of neighbouring hospitals, community trusts and local health groups in vaccinating staff and other priority groups
  • there will also be vaccinations sites across the country

The government has purchased 7 different types of vaccine in advance and procured 355 million doses. This includes 100 million doses of the vaccine being developed by AstraZeneca and the University of Oxford.

The UK government is working closely with the devolved administrations to ensure an aligned approach to COVID-19 vaccine deployment across the UK.

Chair of Vaccines Taskforce, Kate Bingham, said: “We are moving ever closer to having the means with which to help end this pandemic but we must wait for the outcome of the MHRA assessment before we will truly know if we have our first approved vaccine.

“Irrespective of the outcome, which we all hope will be positive, this is a tremendous day for science. It is testament to the Herculean efforts of the scientific and biopharmaceutical community and it makes me immensely proud.”

The do’s and don’ts of snacking

Healthy Snacking at home

With more people staying at home during the working day, due to COVID-19, the temptation to reach into the snack cupboard and graze on less healthy food, such as crisps, biscuits, chocolate, sweets and cake, may be high. Regularly snacking on foods that are high in energy (calories), fat, sugar and salt can increase your risk of developing heart disease and type 2 diabetes.

Here’s some simple tips for making healthier snacking choices at home:

Read the labels

When buying packaged snack foods, check the nutrition label on the pack and aim to choose foods that have:

  • 6g or more of fibre per 100g
  • 5g or less of total sugars per 100g
  • 3g or less of fat per 100g
  • 1.5g or less of saturated fat (saturates) per 100g
  • 0.35g or less of salt per 100g

As the weather gets cooler, you could start your day with a comforting bowl of warm porridge. Rather than adding sugar, honey, jam or syrup to your porridge, you could try adding fruit, such as berries, chopped apple or sliced banana and a sprinkling of chopped nuts, seeds or cinnamon.

The Do’s of Snacking

  • Remove the temptation to snack on foods that are high in fat, sugar and salt by keeping them out of your home. Empty your cupboards of chocolate, crisps, biscuits, cakes and sugary drinks and replace them with healthier options
  • Include a variety of different snack foods in your diet. Choose a range of healthy foods to snack on, such as portions of fruit, raw vegetable sticks, low-fat dips and spreads, wholemeal toast, unsalted nuts, low-fat yoghurts, 30g portions of reduced fat cheese and wholegrain crackers or oatcakes
  • Choose snacks that contain at least one of your 5 a day. Fill your fruit bowl with your favourite fruits and keep plenty of veg in the fridge
  • Choose a portion of fresh fruit to snack on, such as an apple, banana, orange or a handful of strawberries, raspberries, blueberries or grapes
  • Choose vegetable sticks, such as carrot, celery, peppers, cucumber or sugar snap peas with low-fat dips, such as hummous
  • Limit fruit juices to no more than 150ml a day as they can be high in sugar
  • Choose snacks that are high in fibre. Wholegrain foods, such as wholemeal toast and crackers, as well as fruit and vegetables, are high in fibre, which is good for keeping your heart healthy. Choose packaged snack foods that contain at least 6g of fibre per 100g
  • Make your own popcorn at home, without adding salt or sugary toppings. This can be a healthier snack for all the family and a fun one to involve the kids with.

The Don’ts of Snacking

  • Avoid grazing

When you are distracted with work or the TV, or when you are feeling bored, it can be tempting to graze in the absence of hunger. If you regularly graze, try setting specific times when you will allow yourself to eat a healthy snack. For example, you could set an alarm for 10:00am and 3:00pm to enjoy a healthy snack in between your meals.

  • Try to avoid overdoing it

Less healthy snacks, such as crisps, chocolate bars, sweets, biscuits, and cakes.
Avoid cheap supermarket offers on less healthy food options, as they can tempt you into buying (and eating) more than you need.

  • Occasional treats

For an occasional treat, choose foods that are individually wrapped, such as a small chocolate bar or a small packet of crisps, rather than larger sharing packs.

  • Limit less healthy snacks

Try no more than two or three portions each week. Portion (serving) size is often given at the back of packaging and this information can help you to limit the amount you are eating.

Scottish carer shares their first glimmer of hope after Mum’s stroke

Glimmer of Hope after Stroke this Christmas

Stacey Miller from Paisley is supporting the Stroke Association’s ‘Hope After Stroke’ Christmas appeal, after her Mum, Andrea, had a stroke at the age of 45. 

Thanks to the Stroke Association in Scotland, both Andrea and Stacey have been attending a Stroke Café that has been a life saver for both.  It’s given them the opportunity to meet others who have been through a similar experience.   The social interaction and peer support has given both them a belief that lives can be rebuilt again– that there is a glimmer of hope after stroke.

Andrea spent hours, days, and months undergoing intensive rehabilitation in hospital uncertain as to what the future may hold.  But when Andrea left hospital six months later, Stacey was amazed with how far she’d come.

Although Andrea still relies on care, she is in her own home and that means the world to her. Having her two daughters and sister come in and look after her care needs is so much easier than being in hospital.   And although Andrea’s communication difficulties have been challenging, she and her family have found new ways to communicate.

And now Christmas is upon us, Stacey is making preparations with her Mum.

Stacey says: “Mum loved Christmas, it was always a big thing for her.  The stroke took away her freedom to make plans for Christmas and go out and about purchasing all the things that gave her and everyone else joy.

“We knew after the stroke Christmas would be a very different affair, but as Mum’s confidence has grown, she is now more involved.  She participates in decisions over plans, presents etc., even if she can’t physically carry them out.  Christmas is always more magical with kids around and Mum is loving the idea of being involved with  my two year-old  putting up the Christmas tree and looking at all the baubles and decorations together.

“It brings a tear to my eye, watching her with Noah John.  It’s as if nothing has changed and her love of life is still there.

“But I’m aware of her ongoing needs.  The Stroke Association in Scotland has been invaluable in offering a sense of purpose both to Mum and me, that life can be normal again.”

Stacey is asking people to make a donation to the Stroke Association’s work supporting people affected by stroke, as they rebuild their lives this Christmas.

Andrea Cail, Director Scotland of the Stroke Association said: “When someone’s life has been shattered by stroke, they may feel all hope is gone. But we also know that people cling onto even the smallest glimmer of hope. This is what powers them on to achieve what many thought would be impossible.”

The charity estimates that there are over 120,000 people living with the effects of stroke in Scotland, while around 15,000 people have a stroke every year.

“This pandemic has had a serious impact on our ability to raise funds through our usual community events and activities. Many people have praised the support they received from the Stroke Association, to help with that first glimmer of hope to rebuild their lives again.

“That support may be found in a call to our Helpline; through the friendship and support of our online community; or the ongoing support of a phone call from one of our lived experience volunteers. It takes time and money and people to provide this help. And that’s why we’re asking everyone across Scotland to donate to the Stroke Association and help give someone the gift of hope this Christmas.”

COVID vaccinations from next month?

The first coronavirus (COVID-19) vaccinations could be given to health and social care staff, older care home residents and those over 80 years old who live in the community from as early as next month – December 2020.

Updating Parliament on the safe delivery of COVID-19 vaccines, Health Secretary Jeane Freeman outlined that the first groups to be prioritised for vaccination have been identified based on current independent clinical advice.

Once these initial priority groups have been vaccinated, those over 65 will be offered the vaccination along with those under 65 who are at additional clinical risk, before moving on to the wider population over the age of 18. 

An agreement has also been reached with the British Medical Association on the terms and conditions of GPs’ involvement in the programme.

Health Secretary Jeane Freeman said: “I want to be clear that safety is paramount in our approach to the COVID-19 vaccination programme.

“The global scientific, research and pharmaceutical community has come together and worked as never before. That is why we are seeing the front running vaccines delivered in months rather than years, but it is not at the expense of safety.

“Any COVID-19 vaccines deployed in Scotland will be used on the basis of compelling public health grounds, and only when the regulatory bodies and those charged with providing independent clinical advice are satisfied in terms of vaccine safety and effectiveness. The groups we vaccinate first will also be those most at risk from COVID-19.

“Scotland has a strong track record of delivering immunisation programmes, but this programme will be the largest of its kind ever undertaken and we will need more than 2,000 vaccinators and support staff by the end of January for the first phase.

“This is a national vaccination programme which will set out clearly the parameters within which NHS Boards will lead local delivery.

“NHS Boards will identify acceptable and accessible locations, both for mass vaccination and for local access.

“Taking account of local population and geography, they will undertake recruitment and deployment of staff, and the management of local vaccination clinics.

“We will be getting in touch with the first priority groups in the coming weeks and I urge people to take up the vaccine when they are offered it.  It offers additional protection that we don’t have by other means.

“I also want to thank those who are not in the first priority groups for their patience as we protect the most vulnerable people in our communities first, in line with the scientific and clinical evidence. 

“The most important thing you can do to protect yourself and others from the virus in addition is to continue to check and follow the Scottish Government’s guidance for your area and above all to follow FACTS.”

Letters: The deadliest common cancer doesn’t stop for COVID-19

Dear Editor,

Around 780 people are diagnosed with the pancreatic cancer every year in Scotland: tragically, and more than half of people with will die within the first three months of diagnosis.

Pancreatic cancer hasn’t stopped because of COVID-19.

We know that during the first lockdown people were reluctant to go to their GP, that’s why during this year’s Pancreatic Cancer Awareness Month we’re doing all we can to raise awareness of the symptoms and, urging anyone whose systems persist, to use the NHS. There’s no time to wait.

If more of us know the symptoms – which include tummy and back pain, indigestion, itchy skin or yellow skin or eyes, unexplained weight loss and oily floating poo – it could lead to more people being diagnosed earlier. Crucially, this could increase their chances of being eligible for life-saving surgery.

I urge your readers to find out more about the disease and if they are worried that they have the symptoms of pancreatic cancer they should speak to their GP as soon as possible.

CANCER: ‘If you’re worried – see your GP and get checked out’

Four thousand fewer pathologically confirmed cancer diagnoses in March – June 2020 compared to 2019

As part of the ongoing response to COVID-19, Public Health Scotland provides an overview of changes in health and use of healthcare services during the pandemic.

This week, data on the numbers of people who have had a diagnosis of cancer confirmed using a sample of tissue (a pathologically confirmed cancer) between 1 January and 21 June 2020, compared with how many there were in the same period in 2019 are included in our analysis for the first time. 

This information is provided to help inform the ongoing response to COVID-19 and is a subset of the fuller information on cancer incidence in 2020, which is due to be published by the Scottish Cancer Registry in 2022.

The data show that between January and March 2020, the number of people who had a pathological diagnosis of cancer was similar to the number in the same period in 2019. 

By the week ending 21 June 2020, around 4,000 fewer people in Scotland had a pathologically confirmed cancer diagnosis than would have been expected.

Between March and June 2020, numbers fell by about 40% of those in 2019. In total, therefore, between January and the end of June 2020, the overall number of patients diagnosed was down by 19% on 2019 figures in the same period. 

The fall in numbers is unlikely to be due to a reduced occurrence of cancer, and is likely to reflect a combination of: patients with possible cancer symptoms not seeking out help from their GP (as they may be worried about catching COVID-19); changes to the national screening programmes in the first few months of the pandemic; or reductions in the availability of diagnostic services and treatment during this period.

It is clear that COVID-19 has caused substantial direct and indirect harm to the health of the people of Scotland.  As we continue to provide guidance on and evidence for actions to stop the spread of the virus we will also monitor the wider impact it has in our communities.

PHS will do this so that, together with partners in Government, local Government and in Health Boards across Scotland, we can work to protect the health of all of Scotland’s people.

Professor David Morrison, Director of the Scottish Cancer Registry at Public Health Scotland said: “Understanding the impact of the pandemic on health and healthcare services is a crucial part of our efforts to respond to and recover from it.

“As part of the Scottish Cancer Registry’s response we wanted to help understand how cancer diagnoses and cancer services have been affected. The reduction in cancer diagnoses this year gives us a sense of the scale of the impact of COVID-19 on all stages of the journey from first seeking help to recovering from treatment.   

“If you’re worried you might have cancer – see your GP and get checked out. It’s probably not cancer – but if it is, the sooner you’re diagnosed, the better your chances of successful treatment.”

WHAT MADE IT BETTER? Edinburgh urged to get on board with anti-bullying drive

People in Edinburgh are being encouraged to support a campaign to address bullying behaviour among Scotland’s young people which has launched during Anti Bullying Week 2020 (16-20 November).

Led by Scotland’s anti-bullying service respectme, #WhatMadeItBetter? launches amid a changing landscape of bullying as a direct result of the Covid-19 pandemic, with 47% of young people in Scotland reporting that they saw or experienced online bullying during lockdown and 59% witnessing an increase in prejudice-based posts, comments and/or attitudes*.

The campaign seeks to empower young people with advice and approaches from those who have experienced bullying, after a consultation with young people and professionals highlighted that the management of bullying related incidents was a key area of concern.

With this in mind, respectme has created a suite of resources for primary and secondary aged children to help approach and guide conversations at a time when learning supervision is has been reduced, while also supporting young people with advice from those who have been there themselves.

Supporting the call is the Deputy First Minister and Cabinet Secretary for Education and Skills, John Swinney, who said: “Anti-Bullying Week is an ideal opportunity to send a clear message that bullying of any kind is completely unacceptable and when it occurs we all have a responsibility to address it.

“Children and young people need to be educated about all faiths and beliefs and learn about tolerance, respect, kindness and good citizenship. We want all children and young people to be able to speak to someone they trust when bullying happens and that is why campaigns like this are so important.

“I look forward to taking part in Anti-Bullying week to highlight to those being bullied, that things will get better, and how we can be that person that did make things better.”

Wendy Harrington, Director of respectme, said: “Young people are heavily impacted by the Covid-19 pandemic restrictions, with every aspect of their lives – from school to home to social activities – affected in some way.

“There’s a wealth of evidence highlighting a rise in bullying behaviour since lockdown, particularly in an online setting, which has no time boundary and is difficult to monitor and supervise. This makes Anti-Bullying Week and the campaign all the more poignant as we seek to start the conversation and share valuable advice.

Speaking out to young people is James Allan, frontman of award-winning Scottish indie band Glasvegas, who shared his personal advice: “As humans, we carry so many experiences with us, and in some way can blame ourselves or believe what other people say.

“If anybody does experience these things, try and challenge that insecurity or doubt in yourself and to know it’s the way we’re wired up. Things will get better and, in the meantime, be yourself, stay true to yourself and believe in yourself.” 

While the campaign launches during Anti-Bullying Week, it will run across the school year until July 2021 to continuously highlight the options available to young people and remind them that things can and will get better.
