Mounting Scottish maternity service staffing shortages spark safety fears, says RCM

Understaffing, staff burnout and fears they cannot provide safe care could see many midwives heading for the exit, according to a Royal College of Midwives (RCM) survey of its members in Scotland. A staggering half of respondents say they rarely have enough staff to provide safe care for women.

The RCM said it is deeply concerned by the survey results, which showed seven out of 10 midwives are also thinking of leaving due to low staffing levels and frustration with the quality of care they can provide. As one midwife responding to the survey said, “I love being a midwife, but I hate the care I am giving.”

The RCM lays out several recommendations to move forward positively and address the issues in the survey. These include a call for more support for midwives early in their career to ensure they feel well supported and positive about their chosen career rather than leaving.

There is also a need to ensure midwives have time to undertake essential education and development which is key to delivering safer and better care. An accurate tool to determine midwifery staffing levels is also overdue and a proportionate and substantial increase in the number of consultant midwives and clinical educators – who play a key role in ensuring safe care.

Jaki Lambert, RCM Director for Scotland said: ‘I know midwives and maternity support workers provide excellent care daily to families, but we cannot rely on goodwill alone.

“This shows deep seated and longstanding issues, certainly worsened by the pandemic which midwives fear are already impacting on the quality of care for women and their babies. There is a worrying catalogue of issues and discontent with an exhausted and a fragile maternity workforce with rock bottom morale.

“They have also been emotionally and physically battered by the pandemic and left feeling that their work and efforts are not valued. We need to move forward together with the Government from this point.

“There is a real disconnect between what maternity services need and what resources are available to them in terms of funding, professional development, resources, and staffing. It is only the incredible determination, skill and sheer willpower of midwives and their colleagues that are holding services up.

“Without action the staff, and the system they are propping up, will break. This is not safe, it’s not sustainable, and it’s not acceptable.”

Fears about understaffing run through the survey. Levels of staffing were consistently described as unsafe, with midwives raising concerns for the safety of women using seriously overstretched services.

Concerns with the quality of care being given was cited by six in 10 of those thinking of quitting.

Women with complex care needs benefit from additional midwifery support but the resources needed to support these women are often not factored into funding calculations, says the RCM.

The RCM says they are concerned that more experienced midwives and those near retirement who delayed leaving to help in the pandemic are starting to head for the exit. The loss of such rich experience impacts on the support available to early career midwives and the students following in their footsteps.

Mandatory training, designed to improve safety in maternity services, has also been a casualty of the workforce crisis, with more than four in 10 respondents unable to do this training during working hours, instead having to do it in their own time or not at all.  

The RCM calls for staff to be given the time to do this vital training. One midwife summed up the situation saying, “mandatory safety training cancelled due to short staffing making a dangerous situation even less safe.”

Growing pressures are also leading to worrying levels of strain and anxiety for maternity staff. Nearly all (88%) in the survey say they have significant levels of work-related stress. One midwife said, “I have nothing left to give my own children and family at the end of the day. I am at breaking point.”

Nearly every respondent said they are missing breaks with over half (52%) saying this happens two to three times a week and has become the norm. Many are not even getting the chance to have a drink or go to the toilet on shifts that can be as long as 12 hours. One midwife reported, “When I say no break, I mean flooding through sanitary products onto my clothing because I cannot take 10 minutes to change my tampon in a 12-hour shift.”

Jaki Lambert added, “The pain and anguish of midwives was very difficult to read about and I hope their words are captured in this survey.  It paints a deeply worrying picture of staff that are on their knees and are not currently seeing a way forward or access to the support they need to deliver the safest and best possible care.

“We can and must do better for midwives, women, babies, and families in Scotland. We know that the Scottish Government wants to fix this and we want to work with them to make sure that midwives are valued and their expertise recognised.

“It is essential that we recruit new midwives into the service, treat the midwives we have better to make sure they stay in post, and that they can continue to develop throughout their careers so that women in Scotland get the maternity care they deserve.”

RCM warn of midwife exodus as maternity staffing crisis grows

Midwives are being driven out of the NHS by understaffing and fears they can’t deliver safe care to women in the current system, according to a new survey of its members by the Royal College of Midwives (RCM).  

The College is warning of a ‘midwife exodus’ as it publishes the results of its annual member experiences of work survey. Over half of midwives surveyed said they were considering leaving their job as a midwife with 57% saying they would leave the NHS in the next year. 

Of those midwives who either have left or were considering leaving, more than eight out of 10 were concerned about staffing levels and two-thirds were not satisfied with the quality of care they are currently able to deliver.  

Alarmingly, the highest level of dissatisfaction among those surveyed came from midwives who had only worked for five years or less in the NHS. This will have a significantly detrimental impact on workforce planning and, says the RCM, the ability of maternity services across the country to provide acceptable levels of safe, quality care. 

The RCM’s General Secretary and Chief Executive, Gill Walton, said: “Every midwife and maternity support worker goes to work to provide safe, quality care. That so many feel that understaffing means they are unable to do so is deeply worrying.

“What these numbers suggest is a midwife exodus, which will leave already-struggling services on their knees. Quite rightly, there is a strong focus on improving maternity safety, but there is a risk that the Government is ignoring the essential ingredient to that: having the right staff, in the right place.  

“Investment and programmes to improve safety in maternity services hang in the balance here, because without adequate numbers of staff with are fighting a losing battle. Every safety report cites understaffing as an issue that is comprising the delivery of safe care, but when are the Government going to take note?

“We are calling on the Government to listen to those best advised and work with us to improve retention and recruitment. All NHS organisations must urgently review their maternity staffing levels using a recognised workforce planning tool to ensure staffing reflects the workload.” 

A report published by NHS Digital in July revealed the number of NHS midwives working in England in May had fallen by almost 300 in just two months. This is the fastest fall for these two months for any of the years listed in the NHS report, which goes back to 20 years say the RCM.  

Burnout among midwives and all maternity staff is higher than ever, particularly after COVID-19 which saw an increase in sickness absence adding to a pre-existing shortage of 2,000 midwives in England alone.

Last week leaked results of an NHS national staff survey showed a sharp drop in those who believe their health and wellbeing is being supported by their employer. This is echoed in the RCM survey, with over half of respondents saying that they did not feel valued by their employer.

Almost all (92%) of midwives and maternity support workers (MSW) said that they did not feel their work was valued by the current Government.

 

Gill added: “Not a day goes by that we don’t hear of a maternity service having to close temporarily, suspend services or divert women to other maternity units just because there simply aren’t enough midwives. This can’t continue because we know it compromises safety and means women don’t always get the safe positive pregnancy and birth experience that they should.  

“It’s also having a shocking impact on maternity staff themselves. A shortage of midwives has undoubtedly worsened rates of physical and mental burnout among our members. Morale among staff is low and has been worsened by the recent pay award in England.  

“Sadly, this survey shows that many midwives and MSWs have had enough. They are feeling very fragile and are simply at breaking point. Enormous demands are being made on midwives and the services they work for, yet investment in these services from the Government remains inadequate to provide the safe, high-quality care that women deserve.

“We have previously advised the Government on what can be done to begin to address this situation which hasn’t happened overnight. Prior to the pandemic the RCM has voiced concerns and solutions to the recruitment and retention issues faced by UK maternity services, and we are happy to do that again, but we are now sadly at crisis point.

That is today we are again calling on the Government and NHS Employers to take action immediate to keep midwives in midwifery before it’s too late.” 

Student nurses return to the frontline

University’s School of Health & Social Care supports testing programme

Scores of Edinburgh Napier nurses and midwives have again rallied behind the national drive to combat Covid-19 by signing up to assist with the asymptomatic testing programme for students.

Around 100 are working at two testing sites in the city between November 30 and December 9, helping their fellow students in the capital make informed decisions about returning home for Christmas and minimising the risk of spreading the virus.

Covid-19 ‘rapid’ tests, available on a voluntary basis, are taking place at University of Edinburgh sites at The Pleasance Sports Complex and St Leonard’s Land. It is recommended that Edinburgh-based students with no symptoms book themselves in for two tests, between three and five days apart.

Students from Edinburgh Napier’s School of Health & Social Care leapt at the chance to assist with the test programme in a non-clinical role, with more than 400 responding to the call for help.

This was narrowed down to around 100, who are covering various shift patterns across the two sites, talking students through the process and providing support and encouragement.

Many of the same students were among 1000 from the School who earlier took up placements alongside NHS workers in hospitals and care homes at the height of the pandemic in the spring.

Marianne Mearns, 30, a final year BN Child Health Nursing student from Livingston, (above) said: “It is an amazing yet scary time to be a student nurse, but I feel proud to be one of the many supporting the university and the NHS during the pandemic.

“Keeping people safe is paramount, and assisting with the testing is a new challenge I am excited to be part of.

“I also undertook an NHS placement at the height of the pandemic and had to stay in a hotel temporarily to protect my fiance who was shielding at the time. It was very challenging but I feel I coped well and it has made me a more competent and confident student nurse going forward.”

Professor Alison Machin, Dean of the School of Health & Social Care, said: “These are challenging times, but I am very proud of our students’ enthusiasm and willingness to apply their skills and get involved. 

“The lessons they learn will stand them in good stead for the rest of their careers.”

Michelle O’Reilly, lecturer in the School of Health & Social Care, said: “This is an experience we are all delighted to be involved in, with our students from across nursing and midwifery demonstrating excellent professional and interpersonal skills in helping to reduce the anxiety of fellow students getting the test.”

Universities and colleges are utilising lateral flow devices (LFDs) – a clinically validated swab antigen test that does not require a laboratory for processing and can produce rapid results within half an hour at the location of the test.

Students will be offered two LFD tests, spaced three days apart which are bookable through their college or university. Those receiving two negative results will be encouraged to safely return home as soon as is practical after the second result.

If either of the lateral flow tests returns a positive result, the student will be asked to self-isolate and undertake a confirmatory polymerase chain reaction (PCR) test through the usual NHS Test & Protect channels.
