Over two-thirds of patients in Scotland say recent NHS changes do NOT benefit them

  • Almost half of respondents desire more same-day appointments (49%)
  • Almost two-thirds (61%) would like to see prioritisation systems to help with hospital backlogs
  • Almost one in five (19%) have seen benefits from the addition of more over-the-phone appointments

Recent research has shown a deepening public concern about NHS services, as only 10% of people in Britain believe ministers are tackling the NHS’s many problems in the right way.

With this in mind and following rising waiting times and growing pressure on the NHS from strike action, healthcare management consultancy, Acumentice, surveyed 1,000 UK adults to explore the public’s opinion of the current healthcare system and how it could be improved in the future.²

Over two-thirds (68%) of patients in Scotland do not believe that any recent NHS changes have benefitted them personally. In particular, text availability with doctors for advice appears to have been the least popular initiative in the region, with no respondents acknowledging the benefit.

In contrast, the survey suggests the most impactful NHS offering for Scotland was the addition of more over-the-phone appointments, with almost one in five (19%) professing they have seen the most advantage to their care from this out of all recent NHS changes.

When questioned about which changes Scotland would like to see from the NHS to improve their experience, the most popular suggestions were: the ability to book appointments online and/or in the NHS app (54%), more same-day appointments (49%) and the option to book double appointments for multiple concerns (41%). 

In addition, over 6 in 10 (61%) respondents felt that using prioritisation systems to ensure that patients with the most urgent needs are seen first would help the backlog of patients waiting for hospital treatment. 

Looking at average perception across the whole of the UK, the survey suggests people desire an updated approach to accessing NHS health services to improve service and efficiency, with a remarkable half of respondents (50%) saying that none of the recent changes already made by the NHS has benefitted them.

Respondents from Wales, in particular (68%), found the least benefit from recent initiatives, while Yorkshire and the Humber were the areas least critical of recent changes (38%). 

Nationally, when asked what they would like to see implemented by the NHS to improve experience, the biggest priority across the UK was A&E waiting times, with nearly half of the respondents (47%) looking for shorter waiting times, followed by an increase in same-day appointment availability (46%) and more flexibility when booking future appointments over the phone (44%).

Interestingly, more than one in four (27%) respondents would like to see the introduction of fines and consequences for cancelled or no-show appointments in a bid to improve the availability of bookings.

Stephen Hall, Director of Consulting Services at Acumentice and former Executive Director for Performance and Planning across Kingston Hospital NHS Foundation Trust and Hounslow and Richmond Community Healthcare NHS Trust, commented on the findings: “This research indicates that, despite recent changes to the NHS, people in the UK believe it has not been successful and have little faith in the future.

“They feel that the approach to NHS care has not changed much in the past and that there is a need for more investment in digital innovation and data-driven healthcare.

“The outcome is that there is undoubtedly scope for a serious debate regarding access to data and analytics within the NHS as it undergoes a period of significant change. It’s also hugely important that digital transformation support plays an increasing role in ensuring these programmes deliver the benefits they should.”

Edinburgh renters amongst the most dissatisfied in the UK

It is a challenging time for renters, with rental prices rocketing and demand far outweighing supply –  a recent study revealed that for every 100 rental ads in Scotland, 197 people are looking for a room to rent. 

And while renters face greater financial hurdles, they are also dealing with more problems with landlords. According to the UK Housing Ombudsman, landlords were issued a record number of complaint handling failure orders between July and September 2022, a 105% increase on the previous quarter.

With this in mind, Online Mortgage Advisor wanted to discover where in the UK and the wider world tenants are most and least satisfied with their landlords. They found that renters in both Glasgow and Edinburgh are some of the most dissatisfied in the UK, despite the implementation of rental controls in Scotland.

How did we do it?

We analysed 276,000 rent-related geotagged tweets across the UK, US, and Europe, as well as other OECD nations, using an academic tool called SentiStrength. 

SentiStrength is an AI tool which detects positive and negative sentiment levels in short pieces of text and assigns them a score from 5 (extremely positive) to -5 (extremely negative).

Key findings

  • Belfast is the UK city with the highest proportion of dissatisfied renters with 43.5% of tweets analysed recorded as negative, followed by Glasgow (41.7%) and Bradford (38.7%).
  • Edinburgh came in at number 7 with 36.1%, meaning two Scottish cities appear in the top 10.
  • The UK ranked 9th overall in the list of OECD countries with 34.8% tweets deemed as negative out of those surveyed, with Sweden, Denmark and Ireland occupying the top three spots.
  • The most commonly mentioned words in negative tweets about rent were: “people”“money” and “time”. 

Renters in Glasgow and Edinburgh amongst the most dissatisfied in the UK

Using the academic tool SentiStrength, Online Mortgage Advisor analysed 276,000 geotagged tweets related to renting to find out which cities had the least satisfied renters.

  • Belfast, is the city with the most dissatisfied renters in the country with 43.5% of all rent-related tweets being recorded as negative. 
  • Glasgow, ranks 2nd with 41.7% of tweets made by renters in the area being recorded as negative.
  • Edinburgh came in at number 7 with 36.1%. 

This means that two Scottish cities appear in the top 10 of our analysis, despite Scotland implementing a rent freeze at 0% from September 2022 until March 2023. The Scottish government then increased this to a cap of 3% in most instances from April 2023. Find the ranking below:

The most common rental grievances

We found it was ‘people’ that tenants take issue with most frequently, with 60 mentions for every 1,000 tweets. Issues range from landlords turning up unannounced to fellow tenants failing to pay their share of the rent and never taking the bins out. The second and third most commonly listed complaints referred to ‘money’ and ‘time’.

Democracy Isn’t Working

In an exclusive poll of 10,000 voters – five times the normal sample size for surveys of voter sentiment  – Tortoise has found British democracy isn’t working.

Twelve years since the Conservatives took power and six since the EU referendum, voters are dissatisfied with their representatives, divided by demography and distrustful of a system that seems stacked against them.

  • Barely half of voters think of Britain as a functioning democracy.
  • Nearly a third want a strong leader unconstrained by parliament when it comes to making big decisions.
  • Three quarters think MPs don’t care much or at all about their everyday concerns.
  • Four fifths feel they have little or no say in how the country is run.
  • 45 per cent of voters would vote to rejoin the EU if there was another referendum, compared with 40 per cent who’d vote to stay out.

RCM warn of midwife exodus as maternity staffing crisis grows

Midwives are being driven out of the NHS by understaffing and fears they can’t deliver safe care to women in the current system, according to a new survey of its members by the Royal College of Midwives (RCM).  

The College is warning of a ‘midwife exodus’ as it publishes the results of its annual member experiences of work survey. Over half of midwives surveyed said they were considering leaving their job as a midwife with 57% saying they would leave the NHS in the next year. 

Of those midwives who either have left or were considering leaving, more than eight out of 10 were concerned about staffing levels and two-thirds were not satisfied with the quality of care they are currently able to deliver.  

Alarmingly, the highest level of dissatisfaction among those surveyed came from midwives who had only worked for five years or less in the NHS. This will have a significantly detrimental impact on workforce planning and, says the RCM, the ability of maternity services across the country to provide acceptable levels of safe, quality care. 

The RCM’s General Secretary and Chief Executive, Gill Walton, said: “Every midwife and maternity support worker goes to work to provide safe, quality care. That so many feel that understaffing means they are unable to do so is deeply worrying.

“What these numbers suggest is a midwife exodus, which will leave already-struggling services on their knees. Quite rightly, there is a strong focus on improving maternity safety, but there is a risk that the Government is ignoring the essential ingredient to that: having the right staff, in the right place.  

“Investment and programmes to improve safety in maternity services hang in the balance here, because without adequate numbers of staff with are fighting a losing battle. Every safety report cites understaffing as an issue that is comprising the delivery of safe care, but when are the Government going to take note?

“We are calling on the Government to listen to those best advised and work with us to improve retention and recruitment. All NHS organisations must urgently review their maternity staffing levels using a recognised workforce planning tool to ensure staffing reflects the workload.” 

A report published by NHS Digital in July revealed the number of NHS midwives working in England in May had fallen by almost 300 in just two months. This is the fastest fall for these two months for any of the years listed in the NHS report, which goes back to 20 years say the RCM.  

Burnout among midwives and all maternity staff is higher than ever, particularly after COVID-19 which saw an increase in sickness absence adding to a pre-existing shortage of 2,000 midwives in England alone.

Last week leaked results of an NHS national staff survey showed a sharp drop in those who believe their health and wellbeing is being supported by their employer. This is echoed in the RCM survey, with over half of respondents saying that they did not feel valued by their employer.

Almost all (92%) of midwives and maternity support workers (MSW) said that they did not feel their work was valued by the current Government.

 

Gill added: “Not a day goes by that we don’t hear of a maternity service having to close temporarily, suspend services or divert women to other maternity units just because there simply aren’t enough midwives. This can’t continue because we know it compromises safety and means women don’t always get the safe positive pregnancy and birth experience that they should.  

“It’s also having a shocking impact on maternity staff themselves. A shortage of midwives has undoubtedly worsened rates of physical and mental burnout among our members. Morale among staff is low and has been worsened by the recent pay award in England.  

“Sadly, this survey shows that many midwives and MSWs have had enough. They are feeling very fragile and are simply at breaking point. Enormous demands are being made on midwives and the services they work for, yet investment in these services from the Government remains inadequate to provide the safe, high-quality care that women deserve.

“We have previously advised the Government on what can be done to begin to address this situation which hasn’t happened overnight. Prior to the pandemic the RCM has voiced concerns and solutions to the recruitment and retention issues faced by UK maternity services, and we are happy to do that again, but we are now sadly at crisis point.

That is today we are again calling on the Government and NHS Employers to take action immediate to keep midwives in midwifery before it’s too late.”
