New partnership for pioneering kidney health research

Clinical trials to accelerate research into treating chronic kidney disease will be strengthened as a result of a new partnership between the Scottish Government, academia and industry. 

NHS Scotland, AstraZeneca and the Universities of Dundee and Glasgow will work together on new medicines to slow the progression of chronic kidney disease, reducing the risk that patients might need dialysis or kidney transplantation.

The partnership will allow for development of new treatments to treat chronic kidney disease, build infrastructure and enhance expertise in renal clinical trial delivery in Scotland with the key aim of improving patient outcomes.

Preventing progression of the disease allows people to live longer, better lives, free from other consequences of the condition such as heart failure, cardiovascular disease and stroke. This allows people to work, contribute to their communities and economy, while reducing pressure on the NHS.

Cabinet Secretary for Health and Social Care, Neil Gray, said: “This project’s ‘triple helix’ approach is a shining example of how collaboration between the NHS, Scottish Government, academia and industry can accelerate health studies. As we navigate the complex issue of chronic kidney disease, our commitment to patient-centred research remains unwavering.

“Together we can advance targeted studies into medicines that slow progression of the disease and reduce the need for dialysis or transplantation, strengthen our workforce through dedicated research fellows and specialised training, provide greater opportunities for patients, and enable Scotland to attract and deliver more innovative trials.”

Total-body scanner set to unlock disease insights

Scotland is set to receive its first total-body scanner in a boost to clinical research that aims to improve the detection, diagnosis and treatment of complex, multi-organ diseases.

The Total-Body Positron Emission Tomography (PET) facility, due to be operational in 2024, will capture images of patients’ entire bodies quicker, in more detail and use less radiation than existing scanners.

In combination with data from the other new scanner in London, the Scotland scanner forms part of a new National PET Imaging Platform (NPIP) which experts believe will accelerate the development of new drugs and diagnostics.

The platform, a partnership between Medicines Discovery Catapult (MDC), the Medical Research Council (MRC) and Innovate UK, aims to advance healthcare research and clinical trials, and unlock new treatments for complex diseases like cancer as well as cardiovascular and neurological diseases.

The NPIP Scotland Total-Body PET facility will allow academics to connect to the new platform to share data and collaborate on research.

The scanners are part of a £32 million investment into the ground-breaking technology by the UK Government, through the UK Research and Innovation (UKRI) Infrastructure Fund.

The facility, based at the Royal Infirmary in Edinburgh, will be jointly managed by the Universities of Edinburgh and Glasgow.

NPIP’s Total-Body PET scanners have higher sensitivity than current technology and will reveal new insights into biology and disease.

PET scanning is a non-invasive imaging technique that can detect diseases’ early onset.

Current PET technology leaves large sections of the human body out of the field of view, requiring the patient to be repositioned multiple times to achieve a full-body view.

Supplied by Siemens Healthineers, the two Total-Body PET scanners will capture superior images of a patient’s entire body in near real-time.

The new scanners are also faster, exposing patients to much lower doses of radiation, meaning more patients – including children – can participate in clinical trials to improve understanding of diseases.

The speed of Total-Body PET scanners mean that NPIP will be able to facilitate more patient scans, enhancing the scale and impact of clinical research projects.

This richer picture of human health will help researchers to develop new diagnostics, improve the quality and speed of drug discovery, and bring them to market quicker to benefit patients.

NPIP’s network of infrastructure and intelligence will provide a complete picture of patients and how they respond to new drugs and treatments.

Uniquely, it will also connect insights from many research programmes and trials. In doing so, it will begin to build a rich bank of data that the PET community can access for the benefit of patients.

New Activate community development course starting next month

LINK UP at North Edinburgh Arts is running an Activate course in partnership with the University of Glasgow.

This is a foundation course in community development. It is open to community members young and old, in the North West Locality. The course is fully funded.

Project investigating the effects of COVID-19 on blood vessels and blood pressure receives Heart Research UK grant

A University of Glasgow project that is aiming to better understand the effects that COVID-19 infection has on blood vessels and blood pressure has received a grant of £250,000 from Heart Research UK.

Research has shown that people who are older, obese, male or those who have other medical problems including high blood pressure, heart disease, diabetes, cancer, or chronic lung conditions, have a higher risk of developing severe COVID-19. High blood pressure is a major risk factor for cardiovascular disease and is very common with more than one quarter of adults in the UK affected.

The virus causing COVID-19 enters the body’s cells through a receptor called ACE2 which is found in the lungs, heart, blood vessels, kidneys, liver, and bowel. ACE2 is very important for maintaining many of the body’s important processes including blood pressure, inflammation, and wound healing.

COVID-19 can also cause damage to the walls of the blood vessels which makes the risk of blood clots higher and this has been seen more often in people with high blood pressure. The reasons for this are not yet known which is why we need to understand more about the links between COVID-19 and high blood pressure.

This study, which will be led by Professor Sandosh Padmanabhan, Professor of Cardiovascular Genomics and Therapeutics, aims to answer whether:

  • High blood pressure makes COVID-19 infection worse and if so, why.
  • COVID-19 infection makes high blood pressure worse and if so, why.
  • Monitoring and management of high blood pressure needs to be a greater priority during the pandemic.

The study will look at routinely collected health records for people in the West of Scotland who attended hospital or had a positive test for COVID-19 between April 2020 and April 2021. This will be compared to the records of patients who attended hospital during 2019, for another reason. They will also look in detail at a group of people with high blood pressure.

Prof Padmanabhan’s team will also study a group of people that have recovered from COVID-19 infection. They will undergo blood pressure monitoring, and tests of heart and blood vessel health. These tests will be repeated after 12 and 18 months to see if there have been any changes. They will be compared to a group of people who have not had COVID-19.

Finally, the study will look at markers in the blood (biomarkers) with the aim of identifying any which are linked with high blood pressure, cardiovascular disease, or death in COVID-19.

This study will give us a better understanding of the links between COVID-19 infection and high blood pressure, and help to improve the long-term outcomes for survivors of COVID-19. Also, the findings may lead to recommendations on the monitoring and management of blood pressure during the pandemic.

Prof Padmanabhan said: “The current COVID-19 pandemic, caused by the SARS-CoV-2 virus, has exposed unexpected cardiovascular vulnerabilities at all stages of the disease.

“The mechanism by which the SARS-CoV-2 virus causes infection is believed to directly and indirectly affect the cardiovascular system potentially resulting in new-onset hypertension, heart failure and stroke and represents an insidious feature of long-COVID.

“The burden of hypertension as a consequence of the COVID-19 pandemic is unknown, but given the scale of the infection especially among the young this will be a major concern for the future. In this project, we plan to generate valuable evidence that will inform hypertension management strategies and reduce cardiovascular risk for survivors of COVID-19.”

University initiative to support young people in Craigmillar

A ground-breaking new collaboration will provide more educational opportunities for young people in areas of Edinburgh and Glasgow traditionally associated with social deprivation.

New learning centres will be established in Craigmillar and Govan, offering educational support to young people aged 7-18. The centres will provide after-school academic tuition, mentoring and programmes to enable young people achieve their ambitions.

The project is a collaboration between the Universities of Edinburgh and Glasgow, in partnership with education charity IntoUniversity. 

The initiative is based upon a shared concern for children growing up in poverty and a belief in the power of education to transform young people’s lives. 

Working closely with local schools, colleges and other agencies, the centres will provide invaluable long-term support for young people, their education and their journey towards employment.

The centres will complement both universities’ widening participation activities, providing community-based support to young people on their doorstep. 

A total of £2.7 million has been raised by the universities to fund the two centres for a pilot period of five years.

IntoUniversity has carried out detailed feasibility work and consultation to ensure that the new centres will play a valuable role alongside other services for young people.

The centres will complement extensive work that both universities already do to widen access to higher education, by providing community-based support to young people on their doorstep. Through activities at the universities young people will develop their understanding of university life and awareness of the opportunities that higher education has to offer.

£2.7 million has been raised to fund the first two centres for a pilot period of 5 years. Further fundraising is underway to enable a third centre to open in Maryhill in Autumn next year.

Lothian MSP Miles Briggs welcomed the initiative. He said: “This is an excellent imitative between Edinburgh and Glasgow University to improve access to higher education for pupils from disadvantaged backgrounds.  

“Being from a less affluent background can put up barriers for young students to go on to further education and I am pleased that this partnership is looking to even the scales.

“The grading fiasco last academic year, where pupils from less affluent backgrounds were marked down by a greater extent than pupils form more affluent backgrounds, highlights the need to improve access to higher education.

“I fully support this new partnership between Edinburgh and Glasgow University, and will support it in whatever way I can.”

Call to improve care for dying people in Scottish hospitals

Marie Curie and the University of Glasgow are calling for action to improve the care and experience of dying people in hospitals. 

Following a series of seminars with leading experts undertaken before the coronavirus pandemic, the organisations found that despite examples of good care, many hospitals are still not identifying enough patients who could benefit from palliative care, and who are unfairly missing out on the right care at the end of life. With hospital services under pressure due to coronavirus this situation is likely to have worsened over the last six months.

With the Scottish Government’s Strategic Framework for Action running to 2021, Marie Curie and the University of Glasgow have offered 12 themed recommendations for consideration by the next Scottish Government, NHS Boards and Integration Authorities.

Maria McGill former CHAS Chief Executive supports the recommendations as her father was cared for in hospital prior to dying at home from heart failure and did not receive any palliative care.

She shares: “Dad was admitted to hospital through the emergency department via 999 ambulance twice in 10 months. There were missed opportunities. Had conversations been started with him, and importantly including mum and me, instead of the rush to discharge him two weeks later, the second admission might have been avoided and they would have both been in a better position to understand what was happening to dad.

“Early discharge planning and engagement with community services is key, but those conversations should involve the family. Our experience was that dad was told he was being discharged and we weren’t involved in the discussion.

“Staff in all settings having the ability to listen, really listen, and I really do understand that for some staff this might cause them anxiety about what they might hear, that they might not know what to do, or have the time to do anything. However, it is so important this opportunity isn’t missed. Even during that second admission 8 weeks before dad died – there wasn’t a recognition of dad’s deterioration.”

Marie Curie and the University of Glasgow are also calling for the Scottish Government to set aside £15 million for a nationwide Change and Innovation Fund to test new models of integrated care involving acute settings. This could help ensure patients get better care, but also improve efficiencies in care across community and hospital settings.

Marie Curie Head of Policy and Public Affairs Scotland Richard Meade said: “Hospitals are absolutely the right place for some dying people to be, but we need definitive action to ensure people get the care they need when in a hospital. More and more people are likely to present with multi-morbidity and complex needs and by 2040 it is expected that up to 31,000 people could be dying in a Scottish hospital.

“While it’s clear that there are some excellent examples of care across Scottish hospitals, we found a worrying level of inconsistency and gaps in care. It’s particularly concerning that knowledge of palliative care among healthcare professionals was patchy with some not understanding that it can be provided alongside curative treatments.”

Professor David Clark and Welcome Trust investigator at the University of Glasgow said: “Over the last ten years our research has consistently shown that on any given day almost one third of patients in Scottish hospitals are in the last year of life and 10 percent will die on their current admission.

“We are not grasping the opportunity to identify these people and to have conversations about their end of life preferences. Covid-19 has revealed the shocking absence of advance care plans in many cases, making it even more difficult to give the right kind of care in pandemic conditions”.

The 12 recommendations from Marie Curie and the University of Glasgow are:

  1. Scottish policy must actively support palliative care in acute settings and appoint clinical and executive leads in each NHS Board for palliative and end of life care.
  2. Hospitals are supported to provide consistently high-quality care that improves patient outcomes and experience.
  3. Everyone with a palliative care need should be identified if admitted to hospital or attending an outpatient appointment.
  4. Hospitals should ensure they have joined-up working across multi-disciplinary teams and specialities to provide palliative and end of life care.
  5. IT and technology must support patient centred care and enable those working with patients to have the most up-to-date information about that care and be able to provide appropriate input.
  6. An increase in the number of palliative care consultants, doctors and specialist nurses.
  7. Scotland should draw upon the power of volunteers to respond to the needs of communities and empower volunteers to support those with palliative needs and approaching the end of life in acute settings.
  8. All acute staff should be given training to provide care and support for those living with a terminal condition, coming to the end of life and dying in hospitals, as well their families, friends and carers.
  9. All hospital staff should have training and support in communication skills to support them in speaking with people with palliative care needs, and their families.
  10. A review of the number of available specialist palliative care beds across Scotland is required.
  11. Hospitals should also review the physical environment to ensure it is appropriate for dying people and their families.
  12. There is a need for robust data and evidence on patient and carer experience, to allow greater scrutiny of care standards.

STEM Academy: New opportunities for lockdown learning

A science education programme is turning social distancing into an opportunity for school pupils and teachers to spend their summer learning online.

The Summer STEM Academy www.stemacademyscotland.org  is making workshops on a wide variety of science and engineering topics available online for free from Monday 15 June.

Led by the University of Glasgow in collaboration with Edinburgh Napier, it features 26 workshops containing videos and activities for teachers, pupils and wider school communities.

All but one involve a video introduction from an expert to a unique project which can be done at home and an explanation of the science behind it. Each of the workshops offer a worksheet to guide learners through activities and experiments such as designing racing cars and measuring the speed of sound.

Volunteers from Edinburgh Napier and the Universities of Glasgow, Edinburgh, Heriot Watt and Strathclyde have crafted workshop activities on topics including robotics, biodiversity and space colonisation.

Edinburgh Napier’s Professor Mark Huxham (above) has organised activities based on biodiversity, and there is also the use of coding to design and control robots with the computer engineering team, and the use of CAD/CAM software to design F1 cars with industry leading experts and Edinburgh Napier engineers in a schools challenge.

Representatives from organisations including the SS Explorer, BioCity UK, Heart of Midlothian FC, the Royal Society of Chemistry and the Royal Academy of Engineering have also contributed their expertise on aeronautical engineering, life sciences, sports science, climate change and the science of sound.

Teachers and students will also have the opportunity to brush up on new science teaching skills with a workshop provided by the Royal Academy of Engineering’s Connecting STEM Teachers initiative. They’ll also have the chance to design their own science teaching resources during a workshop provided by Timstar UK and the WF Education group.

Andrew Gallacher, Head of Teacher Education at Edinburgh Napier, said: “Even three months ago the idea of organising a major Science, Technology, Engineering and Maths event and placing it online would have seemed ambitious.

“However, this is exactly what we have done with the Summer STEM Academy. It represents a fantastic opportunity for pupils, teachers, academics and business innovators to explore stimulating workshops that are structured to offer blended learning and enthuse participants of all ages.

“Such is the global appetite for such an event that the organisers have now received countless information requests from many countries that include Japan, South Korea and the USA.”

The Summer STEM Academy was launched in 2018 and brought high school pupils from 13 local authorities to locations in and around Glasgow, along with registered and probationer primary and secondary teachers.

Follow-up interviews with participants found that both pupils and teachers were significantly more engaged with science, and a reduction in anxiety about both learning and teaching STEM subjects.

Dr Margaret Ritchie, of the University of Glasgow’s School of Chemistry, has organised each of the Summer STEM Academy events and led the effort to put this year’s projects online.

Dr Ritchie said: “The Summer STEM Academy programme was built from the ground up to create quality partnerships between universities, schools and businesses and find new ways for them to benefit from working together.

“It gives students the chance to see academia and industry up close, student teachers the chance to learn new methods of teaching, and academics and industry professionals the chance to reach out to younger people and shape their understanding of how science and business intersect.

“The coronavirus turned our plans for the third year of the Summer STEM Academy upside-down, but it’s also given us the opportunity to open up our workshops and reach a much bigger audience by putting them online. We also hope that it will keep participants engaged and interested in science while schools remain closed, and ensure they’re motivated to learn when schools reopen.

“Our content comes from volunteers across a broad spectrum of backgrounds, from an undergraduate student through academics to industry veterans. They’ve given us a lot of great content for school pupils and student teachers alike, and we’re really excited to make it available for them to explore over the summer.”
