Private warning as former Health Secretary appointed to ‘help government fix health and care’

Alarm Bells: Alan Milburn joins the Department of Health and Social Care’s board to ‘support the government’s ambitious plans for reform’

Alan Milburn has been appointed Lead Non-Executive Member to the board of the Department of Health and Social Care.

The former New Labour Health Secretary has a ‘proven track record of reducing waiting lists and improving satisfaction in the NHS’.

Milburn is also a strong advocate of private healthcare involvement in the NHS. Back in 2015, Milburn intervened in the British election campaign to criticise Labour’s health plans, which would limit private sector involvement in the NHS. Milburn was criticised for doing so while having a personal financial interest in the private health sector.

The current Labour government says the NHS is broken and it is the mission of this government to fix it and make the health service fit for the future. As part of this national mission, experts are being brought in to help develop policy, and NHS staff and patients have been invited to share their experience and ideas to change the NHS at Change.NHS.gov.uk.

Members of the department board provide independent advice and expertise to inform the department’s strategy, performance and governance and the Lead Non-Executive Member provides additional support to the Secretary of State for Health and Social Care in his role as Chair of the board.

The Labour government says that, as a former Secretary of State, Alan brings experience at the highest levels of helping transform the health and care system – but health trade unions will be very wary of Milburn’s appointment.

Health and Social Care Secretary Wes Streeting said: “As Secretary of State, Alan made the reforms which helped deliver the shortest waiting times and highest patient satisfaction in the history of the NHS.

“This government has inherited a broken health service with some of the longest waiting times and lowest patient satisfaction in history. I am delighted to welcome Alan to the department board, where he will offer advice on turning the NHS around once again.

“His unique expertise and experience will be invaluable and he has an outstanding track record of delivering better care for patients.”

Lead Non-Executive Director Alan Milburn said: “I am delighted to be appointed to this role.

“Having spent three decades working in health policy, I have never seen the NHS in a worse state. Big reforms will be needed to make it fit for the future.

“I am confident this government has the right plans in place to transform the health service and the health of the nation. I’m looking forward to working with them to achieve that mission.”

Due to ‘the requirements of the role and the unique expertise and experience Alan Milburn brings’, he was appointed directly by the Secretary of State on following consultation with the Commissioner for Public Appointments, and in compliance with the Governance Code on Public Appointments.

The shape of things to come? UK Government boosts use of private sector to cut NHS waits

Thirteen new community diagnostic centres are opening across England to deliver more than 742,000 additional scans, tests and checks a year

  • The Elective Recovery Taskforce – formed last year to identify ways to cut waiting times – publishes plan to maximise independent sector capacity to treat NHS patients more quickly
  • Measures include better use of data to help the NHS identify potential opportunities for the independent sector to support patient care, and expanding training opportunities for staff

Thirteen new community diagnostic centres (CDCs) – including eight independently run CDCs – are being launched across England as part of UK government plans to use the independent sector to cut NHS waiting lists, Health and Social Care Secretary Steve Barclay will announce today.

Five of these independent sector-led CDCs will operate in the South West of England, with permanent sites fully opening in 2024 in Redruth, Bristol, Torbay, Yeovil and Weston Super Mare. Additional diagnostic testing capacity is already being rolled out in the region via the use of mobile diagnostic facilities, to provide additional diagnostic services while these sites are constructed.

Three others will open in Southend, Northampton and South Birmingham – with the former commencing activity from November and the latter two from December. These independently run CDCs will help to make it easier for patients to receive checks closer to home and will remain free at the point of use for patients. This adds to the four CDCs run by the independent sector that are already operational in Brighton, North Solihull, Oxford, and Salford.

Five more NHS-run centres will also open across the country, delivering on our ambition to open up to 160 across the country by 2025, backed by £2.3 billion. These will be in Hornchurch, Skegness, Lincoln, Nottingham and Stoke-on-Trent.

Health and Social Care Secretary, Steve Barclay, said: “We must use every available resource to deliver life-saving checks to ease pressure on the NHS.

“By making use of the available capacity in the independent sector, and enabling patients to access this diagnostic capacity free at the point of need, we can offer patients a wider choice of venues to receive treatment and in doing so diagnose major illnesses quicker and start treatments sooner.

“The Elective Recovery Taskforce has identified additional diagnostic capacity that is available in the independent sector which we will now use more widely to enable patients to access the care they need quicker.”

As well as being more convenient for patients, CDCs drive efficiency across the NHS by shielding elective diagnostic services from wider hospital pressures.

The government has also set out a range of new measures to unlock spare capacity within the independent healthcare sector. This comes following actions from the Elective Recovery Taskforce which was established last December.

Chaired by Health Minister Will Quince and made up of academics and experts from the NHS and independent sector, the taskforce looked for ways to go further to bust the Covid backlogs and reduce waiting times for patients.

The measures include a commitment to using data on independent sector providers to identify where they have capacity to take on more NHS patients to help clear the backlog and increasing the use of the independent sector in training junior NHS staff.

These thirteen new CDCs will provide capacity for more than 742,000 extra tests a year once all are fully operational, bolstering access to care.

Independent sector led centres will function like NHS-run CDCs, but staff will be employed by the independent sector, which also owns the buildings. The South West network will be run by InHealth, a specialist provider of diagnostic tests which has worked with hospitals and commissioners across the health service for more than 30 years. By utilising independent sector staff, the NHS will be able to keep pace with rising demand in the region and deliver a high number of tests for patients.

There are currently 114 CDCs open across the country, which have delivered an additional 4.6 million tests, checks and scans since July 2021. Alongside this, significant progress has already been made to cut waiting lists, with 18-month and two-year waits virtually eliminated.

Health Minister and Elective Recovery Taskforce Chair, Will Quince, said: “We have already made significant progress in bringing down waiting lists, with 18 month waits virtually eliminated.

“I chaired the Elective Recovery Taskforce to turbocharge these efforts and help patients get the treatment they need.

“These actions will bolster capacity across the country and give patients more choice over where and when they are treated.”

The taskforce aims to form strong local relationships between NHS organisations and the independent sector. This will help to support improved training opportunities for junior doctors through first-hand experience of procedures. This follows the NHS Long-Term Workforce Plan which will deliver the biggest training expansion in NHS history and recruit and retain hundreds of thousands more staff over the next 15 years.

The department has also published its response to a consultation on a new procurement system known as the Provider Selection Regime, which will give commissioners of healthcare services more flexibility when selecting NHS and independent sector healthcare providers. This is intended to remove unnecessary levels of competitive tendering and barriers to integrating care, which will help to promote collaboration across the NHS and wider healthcare system.

NHS England will evaluate the independent sector’s impact on healthcare capacity and has already begun publishing regular monthly data on independent sector use, showing its contribution to tackling the backlog.

NHS England National Clinical Director for Elective Care, Stella Vig, said: “Hardworking staff across the NHS have made significant progress towards recovering elective care, and it is testament to their efforts that widespread innovative measures are already being rolled out to transform our services and bring down the longest waits for patients.

“Alongside this, we have increased our use of the independent sector by more than a third since April 2021  – carrying out 90,000 appointments and procedures every week, including more than 10,000 diagnostic tests – and independent providers will continue to play a key role as we work towards the next milestone in our recovery plan, as well as the additional one stop shops announced today as part of NHS England’s rollout of community diagnostic centres.

“As this report details, we have already made significant progress in this area, including operating mutual aid systems across both the NHS and independent sector, and by expanding My Planned Care to make it easier for patients to choose where they receive care.”

David Hare, Chief Executive of IHPN, who sat on the Taskforce, said: “The publication of this report is good news for patients. This is a real, significant step forward to unlocking more of the capital, capacity and capability of the independent sector.

“Today’s report builds on the Prime Minister’s recent welcome announcements about how the government is committed to providing patients with better choice over who provides their NHS care, as well as positive changes in how services are procured, which can help add overall capacity and speed up waiting times for NHS patients.

“The report’s commitment to open further independent sector-led Community Diagnostic Centres is also good news for patients, deploying some of the private capital that is available to build new facilities and to help ensure that more NHS patients can get the tests and scans that they need.”

Rachel Power, Chief Executive of the Patients Association, said: “We are advocates of patients having choice and welcome today’s announcement. In particular, the news that GPs will tell patients, at the point of referral, of options for treatment other than the local hospital or clinic.

“Patients in England already have a right to choose where they are treated but not all patients are aware of this right or exercise it. Our expectation is that once GPs offer patients a choice of where to receive treatment, more and more patients will choose to travel further to receive treatment if that means shorter waits.”

Justin Ash, CEO of Spire Healthcare, said: “The best way to cut waiting times for patients is for the independent sector to be fully integrated as part of the solution, and to offer patient choice.

“We welcome the Elective Recovery Taskforce’s recognition of this and are pleased that it has recommended some bold and far-reaching steps to encourage collaboration, promote patient choice and engage the independent sector to help deliver the NHS Long Term Workforce Plan.

“The Taskforce’s work will genuinely benefit patients, who’ll be able to choose where they can receive treatment most quickly, regardless of whether that’s at an NHS or an independent sector hospital.”

This builds on previous work to give patients greater choice. At the point of referral (for example, at a GP appointment), patients will be actively offered a list of providers which are clinically appropriate for their condition.

This will be a minimum of five providers where possible. And by October 2023, all patients waiting over 40 weeks who have not had a first outpatient appointment booked or where a decision to treat has been made but the patient does not have a date for their treatment will be able to initiate a request to transfer to another provider and receive treatment more quickly.

Last month, the Health and Social Care Secretary also convened ministers, clinical leaders and health experts for the NHS Recovery Summit to collaborate and drive forward ideas to help cut waiting lists and improve care for patients.

Rightly so, the NHS is looked upon as a public service, an absolute essential for the vast majority of people living in the UK. It took many decades of struggle and activity against conservative opposition to get this principle established and brought into being by the Labour Party supported by the Trade Unions and the unity of the people.

As a public service the NHS is literally a lifeline: how on earth would people cope with the financial cost of private treatment? They just could not.

There are other essential services needed by everyone every day to be able to function.

First, every home need power: electricity or gas for heating and cooking. Many households cannot do both properly as it is too expensive, yet millions of pounds aer made by investors in these private companies. They should be publicly owned and the millions made used for the benefit of consumers.

The same applies to the essential industries of passenger transport;, rail, bus and tram. Millions are made by investors out of people having to use them just to get to and from work. As essential servces they too should be publicly owned.

All these issues are common to all and as necessary as the NHS. To achieve these ojectives, pressure and demand must be developed showing how the greed of the few wealthy investors affects us all.

Dear Editor

Particularly during and since the Thatcher times people have been encouraged to think only of themselves. It has the divisive effect – which was it’s intention – of tearing people away from their natural instinct of caring for others. The evidence that the policy of looking after number one is the wrong thing to do is all around us, those with the most wealth have the most power.

A divided people are easier to manipulate: those in work against the unemployed who have to exist on benefits, the disgusting campaign against the disabled living on benefits, pitting those who work in the private sector against those in the public sector, the disgraceful ‘bedroom tax’ action taken against tenants and the continuing privatisation of public property …

The situation is not unique to this country; exactly the same is happening in many parts of the world, demonstrating that the capitalist system under which we live cannot solve their problems. What can we do? We can make a start to change things by identifying essential needs everyone has in common, to maximise unity; these essential common needs will show that no individual or groups of individuals should have the power to restrict or withhold them in the pursuit of profit. The following is a most obvious list of common needs upon which that unity can be achieved and removing them from those who only see those needs as a source of private profit:

  • Heating and power: this basic necessity should be treated as such, not as a source of private profit
  • Water & sewerage services: As above, absolute health essential
  • Transport: Essential public service to enable easy access to and from work and social activities
  • National Health Service: To ensure treatment is a permanent public universal service paid for out of taxation.
  • Recreation and Learning facilities of all kinds: Work is not an end in itself but a means of developing an intelligent quality of living in whatever choice of interest.
  • Greenbelt and Open Spaces: Selling these off to developers to be stopped immediately and lawful public consultation on any building applications.

Government pushes ahead with Royal Mail sell-off

postalQueen of the privatisers Margaret Thatcher thought the better of doing it, then New Labour’s Peter Mandelson tried but failed to do it but now it seems that it’s going to be third time unlucky as the coalition government moves to sell off the Royal Mail … a national asset that belongs to all of us:

Business Secretary Vince Cable told MPs yesterday: “Now the time has come for government to step back from Royal Mail, allow its management to focus wholeheartedly on growing the business and planning for the future. It’s now time for employees to hold a stake in the company and share in its success. This government will give Royal Mail the real commercial freedom it’s needed for a long time.”

Around 150,000 staff will be offered free shares when Royal Mail – one of the world’s oldest postal services – is sold off. It’s expected that the company will be worth snywhere between £2.5 – 4 billion when it is floated on the stock market later this year

Mr Cable said privatisation was necessary to ensure that universal service, which currently guarantees delivery to all parts of the country six days a week, can continue. Th government also says that the sale will give Royal Mail the access to private capital it needs to grow and remain competitive.

Royal Mail is currently refocusing it’s business priorities, targetting parcel delivery to cash in on the rapid growth of internet shopping as the number of posted letters falls due to the explosion of email. This change of emphasis saw Royal Mail more than double its profits last year after years of losses.

Ed Davey, minister for postal affairs, said the proposals ‘safeguard the future of both Royal Mail and the Post Office – two cornerstones of British life’, but there are many who are question whether the sell-off is either desirable or necessary – and industrial action looks likely.

Billy Hayes, general secretary of the Communication Workers Union, said: “The fact of the matter is the British public don’t want to see the Royal Mail privatised. Vince Cable is flogging this company to the very people he denounced at the Liberal party conference – the spivs, the speculators and those who just want to make a fast buck.” He went on: “Royal Mail is thriving in public ownership, increasing its profits, providing good quality services and decent jobs. We want it to remain that way. Privatisation would be a throwback to the tired old politics of the 1980s.”

Dave Ward, CWU’s deputy general secretary, said he expected members to be balloted before the end of September. “We will be balloting for strike action, we’ve already adopted that policy. I expect that policy to be ratified by the conference of the union at the end of July. That’s about our members’ terms and conditions, I want to make that clear. It’s about what we fear will happen with Royal Mail as a private operator – what we want is a legal binding agreement that protects their terms and conditions, their contracts of employment, their pensions, for the foreseeable future.

“I don’t think our members will be bought off by the free share issue,” he added. “I believe our members are too long in the tooth not to know the dangers of privatisation.”

Speaking after the announcement North and Leith Labour MP Mark Lazarowicz said: The Government has nationalised Royal Mail’s liabilities in the form of the pension fund in which there was a large shortfall and is now intent on privatising the profits. It argues that it is necessary to subject Royal Mail to commercial discipline and give it access to private sector capital but Royal Mail’s overall operating profits more than doubled over the last year from £152m in 2011-12 to £403m in 2013-13!

“A privatised Royal Mail might continue to operate the universal service provision of delivering 6 days a week to anywhere in the UK for the same price but for how long if its commercial rivals don’t have to? As with the East Coast Main Line, ideology seems to have won out over common sense – it will be private investors who benefit from its commercial success not the taxpayer or customers.”

City campaign groups denounce ‘back door privatisation’

LOCAL CAMPAIGN GROUPS DENOUNCE “BACK DOOR PRIVATISATION” IN CLEANSING

Local anti-cuts groups have declared that Edinburgh Council are responsible for the deteriorating service in refuse collection, and the blame should be put firmly on the councillors and top management, not the binmen and other workers. The groups denounce the “back door privatisation” of Edinburgh Council services and have demanded a response from Council leader Andrew Burns.

North Edinburgh Fights Back and Greater Leith Against the Cuts condemn the large-scale use of agency staff in Street Cleansing and Refuse Collection – and claim that the new switch to fortnightly collections appears to be motivated by cost-cutting not environmental concern.

“We are in favour of reducing waste and increasing recycling but we suspect the move to fortnightly collections is not motivated by ecology but is part of the cut-backs. We are closely monitoring its impact and are consulting with local people if action needs to be taken to restore the weekly collections.”

The groups also denounce the imposition of anti social shift patterns on the workforce, arguing that the continuation of work until 10.30pm will cause disruption and nuisance to residents, particularly children, the elderly and vulnerable. “Refuse collection workers have told management the new shift patterns would not work,” say the community groups, “but as usual the CEC management aren’t listening to the people who really know the job.”

In a statement, the groups declare:  ‘We are totally opposed to the privatisation of public services and the cuts in services and worsening working conditions this invariably entails. The full Council voted against privatising these services – why are they now doing the complete opposite, and bringing in large numbers of Blue Arrow contractors to both Street
Cleansing and Refuse Collection?  New workers – who are certainly needed – should be taken on as full Council employees, wherever possible with permanent contracts.’

They add: ‘Contrary to some claims, Blue Arrow were NOT introduced to deal with the current backlog, dozens of Blue Arrow staff have been operating from the Russell Road depot in refuse collection since March, and Blue Arrow have been deployed from Cowans Close depot in street cleansing since July. What’s more, the private firm Enterprise were used in street cleansing in the spring.’

A Shop Steward from within Environmental Services said: “I have repeatedly voiced my concerns at the use of agency staff to City of Edinburgh Council Senior Management at numerous meetings since the elected members rejected Privatisation and also to various elected members all to no avail; unfortunately it appears that this new administration is
also listening to the officials who are now intent on privatising our council/public services through the back door.”

The community groups are still awaiting a response from the Council. “We have written to Councillor Burns demanding an end to this back door privatisation – he has yet to reply. However Councillor Hinds has written to say she has commissioned a report on the matter from Director of Services for Communities Mark Turley. We eagerly await this and urge its early delivery.”

Campaign group seeks answers on ‘privatisation’

Local campaign group North Edinburgh Fights Back has written to city council leaders seeking assurances that council services will not be privatised. The group wrote the letter following a recent meeting in Muirhouse where concern was expressed over the introduction of increasing numbers of non-council staff into cleansing and refuse operations.

Writing to council leader Councillor Andrew Burns, Mike Vallance said:

I am writing on behalf of North Edinburgh Fights Back, a community group
involved in opposing public service cuts and privatisation, and fighting  against injustice and inequality. You will of course be familiar with our group, not least from our recent presentation of the Anti Poverty Charter to the full CEC meeting.

 As is made clear in the Charter, our group is totally opposed to the privatisation of public services and the cuts in services and worsening in working conditions this invariably entails. As such we played a full part in the campaign to keep the Street Cleansing, Refuse Collection, Janitorial and related services in-house, and were very pleased when the CEC voted to reject the privatisation of these services.

Of course the Labour Group voted to keep these services in-house, and thus our members were shocked when they learnt at our meeting on 25 July in Muirhouse that your administration is presiding over the introduction of large numbers of workers from the private contractor Blue Arrow into the Street Cleansing and Refuse Collection services.

We understand that over the last fortnight around 60 Blue Arrow staff have started operating out of the Cowans Close depot, carrying out street cleansing in Edinburgh city centre. This follows the recent similar controversial use of employees from the private company Enterprise in street cleansing, an episode which was publicised in the local media.

And for several months, refuse collection workers have told us, at least 20 – 30 Blue Arrow employees have been working at the Russell Road refuse depot. In addition five lorries, each staffed by 3 Blue Arrow workers, have been operating out of Powderhall depot. What’s more, Blue Arrow recently advertised for 3 refuse collection supervisors to work on a council contract.

Our members are shocked to hear the CEC are effectively disregarding the decision to keep these services in-house, a decision that so many Edinburgh citizens campaigned to achieve.

We understand from workers we have spoken to, that they believe that new workers are indeed needed and that they should be recruited as normal directly employed council workers, where possible with permanent contracts, and we totally support this.

We have also been informed that shop stewards have brought these matters up with both management and with councillors, and they report their views seem to be ignored.

We also have been informed that the CEC is trying to impose new shift patterns on workers in Refuse Collection, including a compulsory back shift, working up to 10.30pm at night. We are in complete support of theworkforce’s opposition to being forced to work such anti-social hours.

What’s more, it is clear that these hours would mean bin lorries operating in residential areas late at night, causing disturbance to residents, including disturbing children trying to sleep before school the next day. Further we understand from workers that this would cause major problems at addresses where the workers, due to the resident’s disability or frailty, need to come into gardens to take out and empty buckets: such residents are likely to be alarmed at workers doing this relatively late at night, at a time when in winter it will be long past nightfall.

In this respect, we can inform you that at our meeting several people reported bin lorries already operating very late at night, even in the early hours of the morning, in north Edinburgh/ Greater Pilton – we presume this is the private contractors which the CEC are using?

We demand that the CEC reverse the decision to use private contractors in Refuse Collection and Street Cleansing and instead take on the new workers needed as normal directly employed council employees, where-ever possible on permanent contracts. We ask for your immediate reply to explain what you are going to do about this matter.

We further ask that you send us full details of the Blue Arrow contracts in Street Cleansing and Refuse Collection, including length of time, number of workers involved, etc. In addition we ask you to inform us who authorised these contracts, for example were they decided upon at CEC committees and/ or the full Council?

The North Edinburgh Fights Back letter has been passed on to Councillor Lesley Hinds, who has responsibility for Transport, Infrastructure and Environment issues. Cllr Hinds has requested information from the Director of Services for Communities and plans to respond to the group when this information is available.

Further we understand from workers that this would cause major problems at addresses where the workers, due to the resident’s disability or frailty, need to come into gardens to take out and empty buckets: such residents are likely to be alarmed at workers doing this relatively late at night, at a time when in winter it will be long past nightfall.

In this respect, we can inform you that at our meeting several people reported bin lorries already operating very late at night, even in the early hours of the morning, in north Edinburgh/ Greater Pilton – we presume this is the private contractors which the CEC are using?

We demand that the CEC reverse the decision to use private contractors in Refuse Collection and Street Cleansing and instead take on the new workers needed as normal directly employed council employees, where-ever possible on permanent contracts. We ask for your immediate reply to explain what you are going to do about this matter.

We further ask that you send us full details of the Blue Arrow contracts in Street Cleansing and Refuse Collection, including length of time, number of workers involved, etc. In addition we ask you to inform us who authorised these contracts, for example were they decided upon at CEC committees and/ or the full Council?

The North Edinburgh Fights Back letter has been passed on to Councillor Lesley Hinds, who has responsibility for Transport, Infrastructure and Environment issues. Cllr Hinds has requested information from the Director of Services for Communities and plans to respond to the group when this information is available.
